Responsibilities:

Coordinate and manage the Minimum Data Set (MDS) process, ensuring accurate and timely completion
Collaborate with the healthcare team to develop and implement individualized care plans for our residents
Conduct assessments and evaluations to identify resident needs and develop strategies to address them
Ensure compliance with federal and state regulations, as well as internal policies and procedures
Provide education and training to staff members on MDS and care planning processes
Participate in quality improvement initiatives to enhance the overall quality of care

Requirements:

Current Registered Nurse licensure in the state of Nevada
Experience in long-term care or a related field
Strong communication, organizational, and problem-solving skills
Ability to work effectively in a fast-paced environment

Current ACLS certification (Adult patients). Current PALS certification (Pediatric patients) or current NRP certification (Neonatal patients) as required by department.
