We are a highly reputable private duty In-Home Care and Assistance Agency in Newton, currently looking for a full-time LPN to join our fast-paced team. The ideal LPN will be working collaboratively with office staff, field caregivers and family members as needed, ensuring our clients receive the best service possible. The LPN’s main responsibilities are as follows but not limited to:

Conducts ongoing and consistent in-person oversight of client’s care delivery and provides follow-up as needed to family members, caregivers and others.
Effectively communicates with senior living professionals and elder care advocates.
Conducts client and family consultations and draws out client and family needs, routines and preferences related to their care plan
Supervises, mentors and supports caregivers
Maintains complete, accurate and timely client records in WellSky
Displays positivity and professionalism representing the company in communities and facilities
Other general office and clerical functions

The ideal LPN should be able to commute to our office in Newton, and travel to neighboring towns as needed.

Coverage area:

Newton, Wellesley, Brookline, Needham, Waltham, Sudbury, Wayland, Weston, Natick, Framingham, Marlboro.

Qualifications:

Current LPN
Use of an insured vehicle
Valid Driver’s license
Experience in Home Care is preferred but not required.
