Nurse Practitioner/Physician Assistant – Post-Acute Care

Overview

Community Health Partners is seeking dedicated Post-Acute Advanced Practice Providers (APP) to join our growing Post-Acute Care team. This is an exciting opportunity for a Nurse Practitioner or Physician Assistant who is passionate about supporting patients through safe, coordinated transitions of care and improving outcomes across skilled nursing and rehabilitation settings. The Post-Acute APP delivers high-quality, patient-centered care across skilled nursing facilities, rehab centers, and other post-acute environments. In this role, you will complete comprehensive assessments, manage acute and chronic conditions, and partner with facility teams, hospital providers, primary care physicians, and families to support recovery, reduce avoidable readmissions, and enhance patient experience. This position plays a key role in ensuring seamless care transitions and strengthening our post-acute continuum.

Responsibilities

In this role, you will:

 

  • Conduct comprehensive assessments, daily rounding, and follow-up visits for patients in assigned post-acute facilities.
  • Diagnose, treat, and manage acute and chronic medical conditions within APP scope of practice.
  • Develop and evaluate individualized treatment plans that promote patient recovery and readiness for discharge.
  • Order, perform, and interpret diagnostic tests as clinically appropriate.
  • Collaborate closely with facility nursing, therapy, case management, and interdisciplinary teams to coordinate effective patient care.
  • Communicate regularly with hospitalists, primary care providers, and family members to ensure continuity and clarity in the care plan.
  • Support safe and efficient transitions from inpatient hospitals to post-acute care settings.
  • Complete timely and accurate documentation in the electronic health record (EHR).
  • Participate in quality improvement efforts focused on readmission reduction, patient experience, and care outcomes.
  • Attend facility case conferences, interdisciplinary team meetings, and quality reviews as needed.
  • Educate patients and their caregivers on diagnoses, treatment plans, medications, and discharge expectations.

Qualifications

Education and Experience Minimum Requirements:

 

  • Completion of a Nurse Practitioner or Physician Assistant program required
  • Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required
  • Prior experience in post-acute care, hospital medicine, geriatrics, primary care, or related setting preferred.
  • Experience working in Skilled Nursing Facility or managing transitions of care is highly desirable.

Licenses and Certifications:

 

If qualifying as a Nurse Practitioner, the following is required

  • NP - Current State of California Nurse Practitioner License required
  • NPF - Nurse Practitioner Furnishing Number required

If qualifying as a Physician Assistant, the following is required

  • PA - State of California Physician Assistant License required

 

  • DEA - DEA Controlled Substance Number required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • Ability to participate in a federally funded program (Medicare and Medicaid) required
  • NPC - National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferred
  • CRNFA - Certified RN First Assistant preferred

Job Description The UCSF Neurology Program seeks a full time Advanced Practice Provider (APP) to help support our program. The UCSF team consists of highly experienced physicians, surgeons, nurses, social workers, researchers and others. Our Neurology program is nationally known for cutting edge Neurology support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term). The APP will care for patients on the inpatient Neurology services at UCSF, Division of Neurology. Specific duties will include rounding with the inpatient team, answering pages from nurses, writing orders, writing notes (admit H and P, discharge summaries, significant event), evaluation and management of patients and working with care team members for discharges. Will also include evaluation and management for recently discharged patients as they are establishing outpatient care. This job will provide opportunity to gain skills in evaluating neurological patients, perform neurological examinations and manage neurology conditions as part of a team-based care model. Responsibilities Participate in managing hospitalized patients in the ICU and floor settings, coordinating with other team members. Hospital Setting: The APP rounds and is authorized to: Examine patients Record subjective and objective findings in the patient chart and report findings to the physician. Check laboratory results, vital signs, intake/ output, pain scale, and report the results to the physician. Order medications on the approved medication list. Provide wound management, examine for signs of infection, order and change dressings, order or remove sutures or staples. VI. Order speech therapy, respiratory therapy, physical therapy, psychological therapy, and occupational therapy, as appropriate. VII.Refer patients to social work services, nutrition service and home health care. VIII.Refer patients and call on consultation from medical specialties. IX Order nutritional supplementation and enteric nutrition. X.Order laboratory tests and radiologic studies. XI.Write transfer orders to appropriate levels of care. XII Removal and placement of chest tubes, internal jugular lines, central lines, intra- operatively placed drains, surgical dressings, and Foley catheters.
