Physiatry Nurse Practitioner (Skilled Nursing Facility)

As a Physiatry Nurse Practitioner, you will partner closely with a supervising Physiatrist and the interdisciplinary care team to manage the medical and rehabilitative needs of patients recovering from surgeries, neurological events, or other function-limiting conditions. You’ll play a key role in improving outcomes by facilitating smooth recoveries, reducing re-hospitalizations, and promoting independence.

Job Requirements:

Active and unrestricted Nurse Practitioner license in the state of Illinois
Board-certified as an Adult, Geriatric, or Family Nurse Practitioner
Master’s Degree in Nursing from an accredited program
Experience in PM&R, geriatrics, internal medicine, or SNF settings preferred
Strong clinical skills in functional assessments, pain management, and rehab-focused medical care
Comfortable working both independently and in collaboration with a supervising physician
EMR proficiency and commitment to timely charting and care coordination

Key Responsibilities:

As the Physiatry Nurse Practitioner, you will serve as a functional medicine specialist in the facility:

Performing comprehensive assessments focused on rehab goals, mobility, and pain
Developing and updating individualized treatment plans alongside the supervising Physiatrist
Conducting regular follow-ups to monitor progress and modify plans of care
Participating in interdisciplinary care team meetings and patient rounds
Collaborating with therapy teams to track rehabilitation milestones and address barriers to progress
Educating patients and families about diagnoses, recovery goals, and functional strategies
Documenting visits, interventions, and outcomes clearly and promptly in the EMR
Supporting discharge planning and care transitions to ensure continuity of rehabilitation progress
Serving as a clinical resource to staff for musculoskeletal and neurologic conditions

Salary & Compensation:

Annual Pay: $105,000 - $130,000 based on experience

Join the University of Chicago Medicine as an Advanced Practice Provider for Inpatient Hematology/Oncology Night Advanced Practice Provider Team. Sign on Bonus Up to $10,000.00 and Relocation Package Available As an Advanced Practice Provider, you are responsible for comprehensive direct patient care management in the hospital setting, you will play a critical role in delivering comprehensive, evidence-based care to critically ill patients in a fast-paced academic medical center environment. The night shift Advanced Practice Provider covers three inpatient services: Inpatient Leukemia, Inpatient Cellular Therapy, and Inpatient Chemotherapy . These services are all APP-staffed services which each have the support of a Hematology/Oncology Fellow and Hematology/Oncology Attending. After an orientation period on both days and nights, the APP will work independently and autonomously on the Inpatient Hematology/Oncology Night APP Team. At night, the APP will be responsible for managing established inpatients and admitting patients appropriate for the services. The APP will be trained to manage oncologic emergencies such as neutropenic fever, tumor lysis syndrome, and cytokine release syndrome. The APP will always have the fellow and attending as resources, although they will not be in house at night. Optimal staffing at night is 2-3 APPs covering all three services per night. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. APP Job Summary: The Nurse practitioner/physician assistant is an APP responsible for comprehensive direct patient care management. The APP manages a caseload of patients and exercises autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The scope of practice also includes family support, teaching, staff education, consultation, and research. APP: Provide comprehensive care to patients in the inpatient settings cared for by the APPs of the cardiac and vascular surgery ICU and stepdown units. Work collaboratively within a multidisciplinary team, using advanced clinical skills to manage a caseload of patients.
