Serenity Hospice is looking to add a dedicated and compassionate On-Call Registered Nurse to delivering exceptional care, comfort and support to patients as needed. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This position will cover the Essex/Hudson/Passaic County area.

Responsibilities:

  • Respond to manage distress calls as needed.
  • Provide nursing care, emotional support and education to patients and families.
  • Complete an initial assessment of patient and family to determine hospice care needs
  • Regularly re-evaluates the patients nursing needs
  • Use assessment data to determine care plan needs
  • Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated
  • RN counsels the patient and family in meeting end of life needs
  • Registered Nurse initiates appropriate preventative nursing procedures
  • Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
  • Supervises the Hospice Aides and LPN's

Requirements:

  • Candidate must be able to work nights including weekends.
  • Candidate must be able to work on-call schedule (one week on, one week off)
  • Current NJ RN License in good standing
  • Hospice Experience Preferred
  • Willing to travel to the assigned areas
  • Must have reliable transportation with liability insurance
  • Strong communication and interpersonal skills.

As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers. Responsible for identifying patient and loved ones/caregiver needs and for providing supportive care in accordance with the attending physician’s orders and plan of care and the policies and procedures. The functions of this position will include the following: Shall be assigned patient caseload by the Patient Care Coordinator or designee Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process Initiate communication with attending physicians, Hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and loved ones/caregivers Maintain regular communication with the attending physician concerning patient and loved ones/caregivers Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support Provide accurate and timely documentation in electronic medical records. Participates in and contributes appropriate information for Interdisciplinary Group meetings Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate Meet regularly with the Hospice nursing staff to review problems or unique issues from caseloads; share professional support; and exchange feedback aimed toward enhancing professional growth Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation Supervise and document the NAC every two weeks or as assigned Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during on-call time or as requested by the Patient Care Coordinator to fill patient/family needs during the absence of other primary nurses Provide appropriate support at time of death and period of bereavement Participate in the Hospice orientation and in-service training programs for professional staff Participate in agency and community programs as required to promote professional growth and understanding of hospice care Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice Accept other assignments as appropriate About You: Possession of a current license to practice as a registered nurse in the State of NJ At least two years of work experience as a professional nurse preferably at least one of the years in hospice care Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
