Hospice Tower Health at Home NP

Job Summary Tower Health at Home is seeking a full time Nurse Practitioner to join their team. The role of the Nurse Practitioner is to address day-to-day clinical needs of hospice patients in the home and hospital setting; Face to face encounters for recertification of hospice benefit periods (virtual or in-person). NP’s role will include addressing symptom management needs; addressing patient concerns; completing F2F visits, as per regulations; seeing GIP patients; helping educate community/staff about hospice. GIP patients may be located at hospitals in Reading, Pottstown, and Phoenixville. Schedule is 8am-4pm on weekdays.
