In an Addiction Medicine setting, the nurse supports the smooth operation of the program while providing direct patient care. This includes organizing daily clinic activities, coordinating patient flow, communicating with providers and support staff, and helping maintain compliance with regulatory and documentation requirements. The nurse also manages medication administration by preparing and administering medications used in addiction treatment, monitoring patients for side effects or clinical changes, maintaining accurate medication records, and offering education and support to patients throughout their recovery process.

Summary

The Registered Nurse – Patient NavigatorPatient Navigator, Addiction Medicinerole assists patients manage the complexities of post discharge care by providing personalized, professional and compassionate care and follow up to patients and in-patient care. The Patient Navigator will work closely with care management and patient care services to facilitate high quality and timely follow up.

Required Minimum Knowledge, Skills, And Abilities (KSAs)
  • Education: Graduate from an accredited School of Nursing required. Bachelor’s Degree in Nursing preferred.
  • License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Current ACLS Certification for critical care departments required. Certifications specific to department policies may be required.
  • Experience: Three years of experience in an acute health care setting preferred. Navigator or case management experience preferred.
