As a Visit Nurse (RN), you will play a key role in supporting the health, safety, and independence of the clients we serve. Your responsibilities will include:
Clinical Visits & Assessments

Perform comprehensive nursing assessments, including recertifications, resumptions of care, discharge assessments, and routine in‑home visits.

Evaluate client progress, response to treatment, and ongoing needs to ensure high‑quality, person‑centered care.

Counsel clients and their families on health conditions, care plans, medication management, and other related needs.

Provide clear, compassionate guidance that empowers families and supports safety and independence at home.

Maintain ongoing communication with the Nurse Case Manager regarding updates to healthcare provider orders, changes in client condition, incident reports, etc.
- Collaborate with the interdisciplinary team to ensure seamless, coordinated care.
