At Home Care Partners provides in-home care for individuals who require “just enough” supportive and personal care to enable them to remain in the comfort of their homes by providing compassionate caregivers to help them with their needs.

 

As a Part-time RN Supervisor, your duties would include;

-       Assessing new patients and creating care plans for our aides to follow for each patient.

-         Orientate Aides with new patients

-         Complete 30-day, 60-day, 90-day, and yearly Supervision Assessments for all caregivers in your assigned county.

-         Patient recertification.

-         Caregiver Check-ins.

-         Assist in teaching PCA certification classes as needed.
