Pay Rate: Up to $115,000

Position Overview

Park Avenue Extended Care is seeking an experienced and dedicated RN Supervisor to lead nursing operations on the day shift. This role is ideal for a strong clinical leader who is passionate about high-quality resident care and staff development in a skilled nursing environment.

Key Responsibilities

Supervise, mentor, and evaluate nursing staff to ensure excellence in resident care
Support onboarding, orientation, and ongoing staff education and in-services
Collaborate with the Unit Manager to develop and coordinate resident care plans
Serve as a liaison between nursing staff and management
Ensure compliance with facility policies, procedures, and regulatory standards
Maintain accurate clinical documentation and reports
Assess residents daily, oversee new admissions, and ensure physician orders are carried out
Respond to medical emergencies and make sound clinical decisions
Investigate and report suspected abuse, neglect, or mistreatment per state guidelines
Counsel staff on performance and attendance as needed
Perform additional duties as assigned by Nursing Administration

Qualifications

Graduate of an accredited School of Nursing
Current New York State RN license
BLS/CPR Certification (AHA or Red Cross)
Infection Control Certificate
NYS Mandatory Child Abuse Reporting Training Certificate

Experience & Skills

Minimum 1 year of nursing home experience with supervisory responsibilities
Strong leadership, communication, and critical-thinking skills
Experience with Sigma Care or similar EHR systems preferred

Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans.
