Responsibilities:

  • Provide direct patient care to residents, including administering medications, wound care, and other medical treatments as needed

  • Develop and implement individualized care plans in collaboration with the healthcare team

  • Monitor and report changes in resident condition to the supervisor or physician

  • Assist with the activities of daily living, such as bathing, dressing, and feeding

  • Support residents with continence issues and provide assistance with medication management

  • Participate in professionally supervised therapy programs and on-site therapy services

  • Collaborate with other healthcare professionals to ensure comprehensive care
