Briarcliff Manor Center for Rehabilitation and Nursing Care is seeking a detail-oriented and experienced MDS Coordinator (RN) to join our team in a hybrid capacity. This role plays a critical part in ensuring clinical compliance, optimizing reimbursement, and supporting high-quality, resident-centered care.

Key Responsibilities

  • Coordinate and complete all MDS assessments (admissions, quarterly, annual, and significant change) accurately and on time

  • Ensure compliance with CMS guidelines, state regulations, RAPs, and triggers

  • Collaborate with the interdisciplinary team to develop and maintain comprehensive care plans

  • Monitor and analyze Quality Measures (QM), Quality Indicators (QI), and CASPER reports

  • Participate in audits, surveys, and regulatory inspections

  • Support discharge planning and care plan meetings

  • Maintain accurate documentation and ensure HIPAA compliance

Qualifications

  • Active, unencumbered RN license in New York State

  • Bachelor’s Degree in Nursing required

  • Minimum 2 years of supervisory experience in a hospital or skilled nursing setting

  • At least 6 months of experience in rehabilitative/restorative nursing

  • Strong background in skilled nursing/nursing home environments

  • In-depth knowledge of MDS, CMS regulations, and care planning processes
