Summary
Performs the nursing process through assessment, planning, implementation and evaluation of safe therapeutic care to patients in the Multispecialty Clinics by performing the following duties.

Essential Duties and Responsibilities

  • Assesses the physical and psychosocial needs of patients.

  • Prepares equipment and aids providers during treatment and examination of patient when required.
  • Maintains awareness of comfort and safety needs of patient.
  • Records significant conditions and reactions and notifies MSC Manager or provider of patient’s condition and reaction to drugs, treatments, and significant incidents.
  • Takes blood pressure, pulse, temperature, pulse oximetry and other vital signs to detect deviations from normal and to assess condition of patient.
  • Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
  • Initiates patient education according to individualized needs of the patient, as prescribed by provider and/or hospital policy including patient and family instruction.
  • Possess knowledge of anticoagulation management and thromboembolic conditions.
  • Possess the ability to review medical history, clinical information, International Normalized Ration (INR) results and the patient’s compliance in regard to anticoagulation management and document findings accurately in electronic health record.
  • Assists providers with medication management in the electronic medical record system.
  • Assists with development and implementation of medication refill protocols.
  • Telephone and in-person triage for symptom management.
  • Assists in the development and maintenance of chronic disease registries.
  • Float to other clinics when needed.
  • Communicates clearly.
  • Establishes and maintains effective, professional working relationships with patients, medical staff, co-workers, and the public.
  • Works with MSC Manager/Director on action plans for process improvement and Quality reporting.
  • Demonstrates System Values in performance and behavior.
  • Complies with System policies and procedures.
