Responsibilities:

Provide direct patient care to a diverse population, including short-term rehabilitation, long-term care, and subacute patients
Develop and implement individualized care plans in collaboration with interdisciplinary team members
Conduct thorough patient assessments and evaluations to identify needs and develop effective treatment plans
Administer medications, treatments, and therapies as prescribed by physicians
Monitor patient progress and adjust care plans as needed
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Maintain accurate and detailed records of patient care and treatment plans

Requirements:

Current RN license in the state of Pennsylvania
BLS (Basic Life Support) certification
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Strong attention to detail and ability to prioritize tasks
