The Avenue at Wooster is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment not commonly found in skilled nursing facilities. From our hotel-like environment to our all-private suites, the Avenue is committed to providing an excellent level of physical, medical, emotional, and spiritual support in an environment of comfort and respect. Our facility provides some of the area’s highest level of rehabilitation, nursing and medical support to individuals needing post-hospital care and services. We are looking for a committed individual with a reputation for being cheerful and friendly as they administer care to our residents.

Nurse would be responsible for tracking all wound care as well as doing weekly rounds with wound doctor.

Interview/assess residents with each of the following:

  • admission/re-admission
  • referral from therapy
  • Quarterly significant change
  • Enter documentation on each resident with all of the above in conjunction with the interview assessment
  • Complete assessments on each of the above

Interview/assess usually includes:

  • ADL assessment
  • side rail assessment
  • Fall risk assessment
  • restraint device assessment
  • Bowel and bladder assessment
  • ROM assessment
  • Maintain restorative programming
  • Restorative: Write orders, enter orders, notify family of orders and enter on the 24 hour report
  • Refer to therapy/from therapy which includes all of the above
  • Coordinate restorative programs with therapy
  • Care plans include, but not limited to:
  • ADL’s
  • restorative programs
  • incontinent of bladder/incontinent of bowel
  • At times: restraints/enablers/alarms
  • Showers

Maintaining shower schedule includes:

  • Ensure shower sheet preference completed, care planned and filed. Information given to Activity Coordinator regarding resident’s preferences
  • Complete shower sheet preference changes as resident desires with ADL care plan and activity care plan updated
  • Collect completed shower schedule every week
  • Organize shower sheets after completed, clear vs issues
  • File shower sheets and shower schedules weekly
  • Update shower schedule weekly and as needed
  • Putting out weekly shower schedules on each unit before the weekend
  • Report and confer with wound nurse regarding skin issues

Shower schedule:

  • ensuring each resident’s name is initialed
  • Constant education of showers needing completed with shower sheets, initials and kiosk charting
  • Schedule showers/bathing in ADL care plans as per resident’s choice and update as needed
  • Change and update shower schedule and care plans as needed
  • Update Social Service Designee if resident needs Podiatrist or any other ancillary service.
  • Bowel and Bladder trackers
  • Putting out new ones on all quarterlies and significant change or change in continence pattern
  • Ensure bowel/bladder tracker put out on admissions and re-admissions
  • Collect all bowel and bladder sheets
  • Assess bowel and bladder sheets to ensure toileting programs as needed.
  • Ensure bowel and bladder sheets are completed, if not completed, complete
  • BM alerts
  • Continuing education of documentation and bowel protocol to nurse’s and STNA’s
  • Monitor BM alerts
  • Follow up with nurses, STNA’s and supervisor as needed
  • Report of alert in morning clinical meeting
  • Education of staff us on-going and as needed on above such as showers, BM’s, Restorative Programs, bowel and bladder trackers
  • Corporate reports
  • Maintain and update daily/weekly on Catheters
  • Maintain and update daily/weekly on Corporative Restorative Report
  • Reports to appropriate staff weekly
  • QOL Meeting

Pre-meeting:

Gather info on restraints/enablers, toileting/catheters, decline in ADL’s/improvement in ADL’s and restorative/therapy

Weekly attendance

Report on:

Restraints/enablers, toileting/catheters, decline/improvement in ADL’s and restorative/therapy

Confer with other teams members regarding residents that are acutely ill, problem areas and quality measures

Care Conference Meeting

Pre meeting:

  • Gather info regarding restorative programs and care plan is correct on all scheduled care conferences
  • Meet weekly and as needed
  • Meet with family and IDT managers to report info, assist with documentation, ensure POC followed and satisfaction of POC
  • UR meeting

Attend weekly

  • Gather info and report info as needed
  • Daily morning meeting
  • meet with all managers
  • meet with clinical managers
  • Go over Dashboard with BM alerts, not eating alerts, shower alerts and decrease in fluid intake alerts
  • Mini QA

Therapy

  • Work closely with therapy for referrals to/from restorative
  • Update therapy if see a problem with resident’s ADL’s
  • Ensure all residents have a Transfer level from therapy and complete in care plan so STNA’s are required to document
  • Interview therapist regarding resident’s tolerance and ability to complete ADL’s
  • Collect orders and care plan as needed

Maintain lists for:

  • Restraints/enablers/alarms with last assessment completed to ensure no assessments missed
  • All Incontinent residents in the building
  • Restorative nursing programs on-going
  • Mechanical lift log being hoyers and sit to stands
  • Catheter log for Foleys and S/P Catheters
  • Monitor clients for case mix with MDS nurses, TSI and quality measures

MDS sections: G, O, H to be completed as per the MDS schedule

  • Work closely with MDS and nursing for audit purposes
  • Cover MDS if off, behind or on vacation
  • Orientation of new staff as needed
  • Helping hands
  • Update Social Service Designee if resident requests Podiatrist, Dental, Audiology services
  • Assist in Main Dining Room as scheduled and as needed.
  • Assist with feeds on the floor as needed
  • Assist floor staff as needed
  • Inservices

Catheters

  • Ensure proper diagnosis
  • Ensure proper care plan

Track/report

  • Complete bowel and bladder assessments on admission, re-admission, quarterly and significant change
  • On-going education of nurses
  • Restraints/Enablers/Alarms
  • Restraint device assessments to be done on admission, re-admission, quarterly and significant change as applicable.
  • Ensure order is correct, correct order as needed
  • Documentation on initiation, quarterly, admission, re-admission and significant change
  • Fill out physical restraint consent forms as needed. If updated family regarding enabler/alarm/restraint will get verbal consent and will document same.
  • Speak with STNA’s, nurses and therapy regarding resident’s progress as needed
  • On call weekend manager per rotation
  • Comply with Scorecard recommendations, education and need as per Corporate Nurse
  • ODH with complaint surveys and annual surveys: work under the discretion of the DON, comply with ODH requests and interview with ODH as needed.
