Job Overview

The Nurse Practitioner, Care Management, provides care management and primary care medical services to high-risk patients. The NP manages the assigned patient panel.

Minimum Requirements

Nurse practitioner with an active, unrestricted CA license to practice medicine by the Nursing Board of California.
Minimum 2 years of experience and training as a nurse practitioner.
Minimum of 1 year experience in a high-risk specialty, Geriatric, Hospice, and/or Palliative Medicine, preferred.
Requires a valid and active license to practice medicine in the State of California.
Previous experience with computer-based systems required; Electronic Medical Records experience preferred.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide primary medical care for adult patients at APLA Health at its medical clinic in Long Beach and other clinical sites as needed. The Infusion for Health Nurse Practitioner Manager will play a crucial role in providing clinical leadership, managing day-to-day operations, and ensuring high-quality patient care within multiple ambulatory infusion centers. DUTIES AND RESPONSIBILITIES Clinical Leadership: Engages in remote and in-person supervision in collaboration with the Medical Director, including virtual case discussions, care-plan development, escalated patient issues & reactions, review of infusion protocols, and documentation audits. Ensure that clinical protocols are being followed, and patient safety is prioritized.
