MDS Coordinator Must be RN/LPN
Job Summary:

About the Role:
As an MDS Coordinator, you will be responsible for completing and submitting accurate and timely MDS assessments, reconciling documentation with orders and therapy, and managing PDPM/RUG coding, care plans, quality audits, and staff training. You will also support surveys and QA/QAPI activities, ensuring compliance with regulatory requirements. Responsibilities:
• Complete and submit MDS assessments (admission/quarterly/annual/SC/discharge) on time
• Conduct resident assessments and reconcile documentation with orders and therapy
• Manage PDPM/RUG coding, care plans, quality audits, and staff training
• Support surveys and QA/QAPI activities
• Collaborate with interdisciplinary team members to ensure high-quality care
Requirements:
• Active Illinois RN or LPN license
• 2+ years of long-term care experience; MDS experience preferred
• Familiarity with PointClickCare or similar EHR; strong assessment/documentation skills
