MDS Coordinator LPN
About the Role:
MDS Coordinator LPN are responsible for coordinating and facilitating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments and developing individualized care plans that meet the unique needs of our residents. This critical role requires exceptional communication and organizational skills, as well as a strong understanding of clinical best practices and regulatory requirements.
Responsibilities:
Coordinate and facilitate MDS assessments, ensuring accurate and timely completion of assessments and care plans
Develop and implement individualized care plans that meet the unique needs of our residents
Collaborate with interdisciplinary teams to ensure comprehensive care and support
Maintain accurate and detailed records, including MDS assessments, care plans, and resident data
Participate in quality improvement initiatives to ensure excellence in patient care
Provide education and support to residents, families, and staff as needed
Monitor and evaluate resident outcomes, identifying areas for improvement and implementing changes as needed
Requirements:
Associate's degree in Nursing (LPN) or Nursing Education
Current LPN license in the state of Pennsylvania
Minimum one year of experience in a long-term care setting, preferably in a MDS Coordinator role
Strong understanding of clinical best practices and regulatory requirements
Excellent communication and organizational skills
Ability to work effectively in a fast-paced environment and prioritize tasks as needed
Strong attention to detail and commitment to accuracy and quality
