Position: Registered Nurse Coordinator - Cleft & Craniofacial
Schedule: Monday-Friday, 8:00 AM - 4:30 PM (No weekends, holidays, or on-call responsibilities)
Overview: The Cleft & Craniofacial Nurse Coordinator collaborates with other members of our multidisciplinary team to ensure seamless, coordinated care for patients and families. This role involves guiding families through treatment plans, preparing for clinic visits, and supporting the clinical team to deliver comprehensive care.

Key Responsibilities:
- Coordinate logistics for each cleft and craniofacial team visit, ensuring providers and patients are prepared.
- Prepare and review patient records, including requesting reports, test results, and pertinent information from community providers.
- Manage clinic flow, assist families with questions, and serve as a resource for team members during visits.
- Lead team conferences, documenting recommendations and understanding each patient’s treatment plan.
- Follow up post-visit to review care plans, clarify instructions with families, provide counseling, and coordinate referrals.
- Manage incoming clinical phone triage and develop educational materials for patients and families.
- Organize educational opportunities for team members, patients, families, and the community.
- Support new team member orientation, data collection, and research initiatives.
- Act as a role model and resource within the team, aligning with the organization’s mission and vision.

Qualifications:
- Requires an Associates of Nursing (ASN).
- Bachelors of Nursing (BSN) preferred.
- Requires Basic Life Support certification through the AHA.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Degree from a nursing program accredited through the ACEN or CCNE.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Two years experience in an ambulatory setting preferred.
- Knowledge of procedures for performing the functions appropriate to the specific clinical area and familiarity with designated pieces of equipment and supplies.
- Requires ability to coordinate a number of functions for a group of patients in an effective timely manner.
- Requires ability to assess patients without face-to-face interaction.
- Requires awareness and understanding of legal implications of patient care activities and performance as a Registered Nurse.
