Job Description:

Ensures safe and efficient admissions and transfers into and within the Allina Health System. Works with all persons needed to evaluate appropriate placement and facilitate rapid transfer including; physicians, transportation providers, and nursing staff at referring hospitals.

Principle Responsibilities

  • Patient placement.
    • Ensures safe and efficient admissions and transfers into and within the Allina Health System.
    • Works with all persons needed to evaluate appropriate placement and facilitate rapid transfer including physicians, transportation providers, nursing staff at referring hospitals.
    • Provides nursing report on incoming patients per department guidelines.
    • Ensures proper patient placement per facility guidelines.
    • Evaluates, processes, tracks, documents, and coordinates patient transfers and direct admissions into and within Allina Health utilizing patient placement guidelines specific to the facility.
    • Facilitates transfers between units and departments.
    • Provides support for physician clinical issues.
    • Facilitates all interactions with excellent customer service skills.
    • Meets performance criteria with the Registered Nurse position.
  • Regulatory and resources management.
    • Ensures clinical resource management and adherence to commercial and regulatory requirements.
    • Evaluates, processes, tracks, documents, and coordinates patient transfers and direct admissions into and within Allina Health.
    • Projects potential challenges on a daily basis for patient placement, based on accessible data.
    • Adheres to compliance regulations.
    • Remains current with knowledge to ensure compliance with regulations pertinent to their position accountabilities.

Required Qualifications

  • Associate's or Vocational degree in nursing if a current Allina Health employee
  • Bachelor's degree in nursing if a non-Allina Health employee
  • 3 to 7 years experience in an acute care setting


Preferred Qualifications

  • 0 to 2 years experience in a charge nurse or telemetry position


Licenses/Certifications

  • Licensed Registered Nurse - MN Board of Nursing required upon hire

Pay Range

Pay Range: $44.64 to $61.76 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.

RN For Transitional Care Unit A Transitional Care Unit (TCU) helps ease patients into dialysis. The first weeks of dialysis involve a great deal of adjustment, along with increased risk of hospitalization and mortality. As a TCU nurse, you will provide patients with a solid foundation to start their dialysis journey. The TCU nurse coordinates the care and education of patients to help them feel better and make more informed decisions about their care, including choice of treatment modality—in-center or at-home dialysis. PURPOSE AND SCOPE: The registered professional nurse Home Therapies RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. Assesses and manages patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols and communicates patient related issues to the physician as needed.
