Position Details

Eisner Health is seeking a Pediatric Nurse Practitioner who will provide services to underserved communities. A successful candidate will be responsible for providing direct patient care, multidisciplinary interaction with support services, and other clinical and administrative responsibilities. Utilizing the guidelines set forth by the American Academy of Pediatrics and the Department of Health’s Child Health and Disability Prevention Program (CHDP), the nurse practitioner shall provide primary care to pediatric patients ages 0-18.

Job Duties

  • The nurse practitioner as the Primary Care Provider (PCP) who has been assigned to the patient, shall see the patient at the earliest opportunity following, whenever possible, specified HMO guidelines for initial evaluation and subsequent visits
    • To take care of the patient's immediate needs.
    • To undertake medical evaluation, including history and physical examination, laboratory, X-ray, referrals, or other diagnostic studies.
    • To evaluate the patient and develop a problem list.
    • To develop treatment objectives and plans.
    • To initiate treatment orders and procedures.
    • To work closely with other staff members in coordinating and obtaining needed services for patients
    • To educate and counsel patients and their parents to promote good health and disease prevention
    • To use and disclose patients’ protected health information only in accordance with the Health Insurance Portability and Accountability Act (HIPPA) standards.
    • To practice within the confines of the Center’s established treatment protocols, and as directed by the supervising physician.
    • To consult with the physician supervisor whenever any question arises as to diagnostic evaluation or medical treatment of any patient
  • The PNP will make every reasonable effort to follow through personally with the patients she/he evaluated initially.
  • Patients will be asked to return for further services only as often as necessary and by appointment whenever possible.
  • A visit form will be recorded in the EHR system, signed electronically, for each patient visit
    • To reflect change in a patient’s condition
    • To indicate any diagnostic studies performed, and their results when they become available
    • To indicate changes in patient’s active problems, diagnosis or treatments
    • To indicate the logic of diagnostic evaluations and treatment plans
    • To reflect any pertinent attitudes or behaviors on the part of the patient or their family towards any aspect of patient care
    • To easily allow continuity of care by other providers
  • Specialty clinic services shall be provided if medically necessary.
  • Recommendations made by subspecialists will be integrated into the patient treatment plan.
  • The patient shall be informed by the PNP as to the general nature of his/her medical problem, the general prognosis and the nature and purpose of the treatment and procedures that are contemplated. Appropriate instructions in self- care shall be given by the PNP to the patient and documented in the patient's medical records.
  • A second opinion referral will be generated if requested by the patient’s family, and deemed medically reasonable.
  • A chronic problem list will be generated and maintained as current as possible.
  • Items in the PAQ will be addressed on a regular basis.
  • All medical providers are expected to promote a cohesive, cooperative working relationship with fellow providers which may include helping with excess patient loads and staying after the scheduled working hours.
  • All providers are expected to maintain excellent professionalism when dealing with patients.
Qualifications

  • License to practice as a nurse practitioner in the State of California.
  • Board certified or board eligible in field of Pediatrics.
  • At least 2 years clinical experience as a nurse practitioner in area of specialty. Knowledge and experience in managed care preferred.
  • Effective communication and interpersonal skills with children and families.
  • Proficiency in Spanish is highly preferred
  • Maintain CPR certification.
