We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe appropriate medications, including controlled substances in accordance with OHLaw.
Order, review and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
and/or the care plan.
Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

Current, unrestricted NP license in the state of OHIO
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience working in a long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team to coordinate patient care.

Position Summary: The nurse practitioner (NP) works in collaboration with a multidisciplinary health care team. The NP is generally service/physician aligned, managing a distinct patient population in the outpatient setting. The NP may be required to perform some coordination of inpatient care and facilitate discharges/outpatient follow up. The nurse practitioner may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Performs in-office procedures as prescribed. Presents patient report, collaborates in plan of care, and follows assigned patient population. What you will need: Nurse Practitioner Master’s Level prepared in an area consistent with the population to be serviced. Current Nurse Practitioner license in good standing. Requires Advanced Registered Nurse Practitioner (ARNP) or Certified Registered Nurse Practitioner (CRNP). Nurse Practitioner must be eligible for a DEA number and eligible for credentialing.
