JOB RESPONSIBILITIES:

Obtains and records initial assessment and establishes priorities according to patient age, symptoms, and psychosocial needs.

Reassesses patients systematically to identify progress and trends that require intervention. Implements and documents timely interventions appropriate to patient needs and recognizes and responds to emergencies.

Makes comprehensive nursing decisions based on the interpretation of facts and evaluations of patient outcome.

Participates in patient/significant other health education and discharge planning.

Coordinates team assignments and makes adjustments based upon patient and unit needs; follows up on delegated patient care tasks for quality and completeness. Utilizes appropriate chain of command for resolution of difficult issues.

Participates in orientation of new staff.

Maintains a neat, organized, and safe environment for patients and staff.

Participates in the unit’s continuous quality improvement plan to improve organizational performance.

Functions as a resource to visitors, physicians, and staff; greeting them promptly and courteously.

Performs other related duties as assigned.

Clearly communicates and continuously supports the Mission and Values of Johnson Memorial Health.

Conducts all activities in compliance with applicable laws, regulations, standards, and Johnson Memorial Health policies and procedures including Blood and Body Substance Precautions.

ADDITIONAL REQUIREMENTS:

Critical Care Unit:  ACLS or ALS certification must be obtained within 6 months of hire. Must successfully complete dysrhythmia recognition test at initial department orientation.

Emergency Department: Must complete ACLS or ALS certification during orientation. Emergency Nurse Pediatric Course or PALS must be completed within one year of hire.

Maternity Center: ACLS or ALS Certification required.  NPR certification is required within 3 months of hire.

EDUCATION, EXPERIENCE AND TRAINING: 

Licensure as Registered Nurse in the State of Indiana required. Minimum of BSN required. BLS or CPR for Healthcare Providers Certification required. PALS Certification required for staff who are assigned to care for pediatric patients.

Manage transitions of care episodes for patients on their panel, including timely completion of structured clinical assessments to identify post-discharge needs, medication reconciliation on behalf of the PCP, and addressal of identified needs directly or via collaboration with other team members. Collaborates patient’s PCP, family/caregiver, Social Worker, Behavioral Health Specialists, and other care team members, as needed to evaluate the individual's needs, goals, and plan of action and ensure care plan progression. Ensure timely documentation of key clinical assessments after admissions, while balancing in-center care team planning meetings. Lead in-person interdisciplinary care planning meetings to ensure effective care coordination and management between providers visits. Perform timely nursing assessments and provide patient education for chronic condition management and transitions of care. Educate patients and families, empowering them in their care, and advocating for their needs. Document visits in electronic health record according to internal standards Other duties as assigned. What are we looking for? Fluency in Spanish or other languages spoken by people in the communities we serve, strongly preferred Current RN license in assigned state is required; Bachelor degree in nursing preferred. Minimum of 6-8 years nursing experience. Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire, unless candidate has 2-3 years of relevant care/case management experience 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health is strongly preferred. Demonstrated skill in motivational interviewing, patient activation, time management, and navigating community and social resources. A flexible and positive attitude Comfort with ambiguity and change High emotional intelligence as evidenced by ability to evaluate/perceive a situation from multiple lenses and understand various perspectives in coming to problem resolution. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
