Bronson LakeView Family Care - Paw Paw (Suite A) in Paw Paw, MI is seeking to hire a Nurse Practitioner or Physician Assistant to join their busy established Family Medicine Practice.

Position Highlights:

  • See full spectrum family practice patients
  • Candidate will work alongside two physicians and two APPs
  • No OB or inpatient medicine opportunity
  • Hospital-employed, Full-time outpatient only position
  • Team Approach with Ambulatory Care Manager, clinical pharmacy support, social worker, as well as nurse triage. 
  • As a Rural Health practice, we will work with the APP to create a schedule to offer appointments as early as 7:30 am or as late as 7 pm at least one day a week. 
  • Patients of the practice have access to an onsite pharmacy, laboratory and imaging. 
  • Nurse Triage service is available after hours and on the weekends and patient call volume after hours is very low.  
  • APP will provide call coverage one evening during the week and share call with 17 additional family care providers for weekend and holiday coverage.   

Qualifications:

  • Successful completion of an accredited Physician Assistant or Nurse Practitioner Program
  • Current unrestricted or pending Licensure in the state of Michigan
  • Strong communication and interpersonal skills

Performs comprehensive and problem focused histories and physical examinations, interprets laboratory and radiologic tests. Interprets and correlates subjective and objective patient data, formulating a diagnosis. Under the direction of the physician, reviews patient findings and develops a treatment plan within the scope of practice. Performs above activities in the outpatient and inpatient settings. o Facilitating timely discharges/transfers to the appropriate level of care.

Clinical Management
• Evaluate and appropriately manage patients.

• Complete appropriate documentation necessary to support:

Location BMH Bronson Methodist Hospital Title Lactation Consultant- BMH PRN Under the supervision of the Breastfeeding Center and Milk Bank System Manager of Nursing, the Lactation Consultant will provide education and support the breastfeeding family so that breastfeeding will be successful. The Lactation Consultant will adhere to our Baby-Friendly Hospital Initiative with implementation of the Ten Steps to Successful Breastfeeding and the International Code of Marketing of Breast-milk Substitutes. The Lactation Consultant functions independently and collaboratively with members of the perinatal health care team to provide assistance, teaching and support to breastfeeding families. The Lactation Consultant will be able to assess medical conditions of nursing mothers and refer to medical providers if necessary for breastfeeding related issues. The Lactation Consultant will evaluate outcomes of newborns and verify growth parameters/development milestones while newborn is breastfeeding. The Lactation Consultant will also provide follow-up to breastfeeding patients during post-hospital care with phone calls and outpatient consults. Employees providing direct patient care must demonstrate competencies specific to the population served. Requires high school diploma or GED Preferably 2 years clinical experience BLS Certification required by completion of core orientation Experience working with new mothers and newborns preferred Teaching experience preferred Preferred International Board Certified Lactation Consultant (IBCLC), or required designation within two years of hire Required Certified Lactation Counselor (CLC) or Certified Lactation Specialist (CLS) • Basic computer skills • Effective verbal and written communication skills Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time.
