Job Description

The Clinic Lead Nurse works in coordination with the Office Manager, Medical Director, and nursing leadership to support efficient daily operations, regulatory compliance, patient safety, and service excellence. This position assists in ensuring the achievement of departmental goals through operational oversight, workflow coordination, quality improvement activities, and support of clinical staff. The Clinic Lead Nurse II serves as a clinical resource and leadership presence within the clinic while supporting primary care, preventive care, wellness services, care coordination, employee health, and occupational health programs provided to county employees, dependents, and retirees.

This position plays a key role in supporting employer-sponsored health initiatives, workforce wellness programs, occupational health services, employee onboarding activities, and community partnerships that advance the health and well-being of the Horry County workforce.

Some Responsibilities Includes but not limited to:

  • Serves as a leadership presence within the clinic and provides support, guidance, and mentorship to nursing and clinical support staff.
  • Functions as a clinical resource for staff regarding nursing practice, patient care standards, occupational health protocols, and clinic workflows.
  • Collaborates with providers, the Office Manager, and operational leadership to identify and implement workflow improvements that enhance patient access, efficiency, and service delivery.
  • Coordinates daily staffing assignments and clinical workflows as delegated by the Office Manager to ensure operational needs are met.
  • Provides patient care, insuring prompt response to patient in basket messages, coordinating follow-up cares and ensuring prompt referrals for patient population.
  • Promotes a culture of teamwork, accountability, service excellence, and patient-centered care.
  • Provides direct nursing care utilizing the nursing process and evidence-based practice standards.
  • Performs nursing assessments, patient triage, medication reconciliation, and clinical documentation.
  • Coordinates referrals to specialty care, diagnostic services, physical therapy, behavioral health, occupational medicine, and ancillary services.
  • Manages referral tracking processes to ensure patients receive timely access to recommended care and follow-up.
  • Supports occupational medicine programs and workplace health initiatives for county employees.
  • Assists with pre-employment evaluations, annual occupational surveillance examinations, return-to-work evaluations, and fitness-for-duty assessments.
  • Performs audiometric testing, hearing conservation screenings, spirometry testing, vision screening, and other occupational health assessments as required.
  • Ensures compliance with MUSC Health policies and procedures, HIPAA requirements, OSHA standards, CLIA regulations, infection prevention guidelines, and applicable regulatory agencies.
  • Participates in quality improvement initiatives, chart audits, patient safety activities, and performance monitoring programs.
  • Promotes a culture of exceptional customer service and patient-centered care.
  • Addresses patient concerns and service recovery opportunities in a timely and professional manner.
  • Supports employee wellness initiatives, health education programs, and preventive health activities.

Minimum Training and Education

Associate Degree in Nursing and a minimum of two (2) years of work experience as a Registered Nurse required. Bachelor of Science in Nursing (BSN) preferred.

Previous ambulatory care experience preferred. Experience in primary care, employee health, occupational health, employer-sponsored health clinics, or care coordination strongly preferred.

RN staff hired on or after July 1, 2013, with an Associate Degree or Diploma in Nursing are required to be enrolled in an accredited BSN program within two years and successfully obtain a BSN degree within four years of the RN hire or reclassification date in accordance with MUSC Health Policy A141.

Required Licensure, Certifications, and Registrations

  • Current South Carolina licensure as a Registered Nurse or compact state licensure required.
  • Current Basic Life Support (BLS) certification required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA-recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
  • ACLS certification if required by clinic operations.
  • Obtain DOT/BAT certification within six months and OHN certification within one year are required.

A Brief Overview The goal of the Tidelands Health's Cancer Center Patient Navigator (PN) is to navigate newly diagnosed cancer patients from the surrounding communities to receive timely, patient-centered care at the TH Cancer Center Network. The PN will engage patients, create a trusting relationship, navigate patients to their TH Cancer Center appointments, and closely communicate with a patient's entire care team based on clearly outlined goals and identified patient needs. The PN will aid patients in the coordination and completion of appointments inside the TH Cancer Center and help decrease barriers to timely cancer care and treatment and to increase access to high quality cancer care for underserved patients. As it is extremely important to identify and address supportive care needs starting at cancer diagnosis, the PN will be assisting in administering supportive care services surveys to all assigned patients, which will help inform the navigator about the patients' symptom burden, illness perception, financial distress, and self-efficacy and direct future navigation and care coordination efforts. The navigator's interventions might include education, appointment reminders, help with insurance, transportation, navigating the Cancer Center, assisting in finding appropriate childcare, interpreting, connecting the patient to psychosocial and/or palliative care teams, and directing the patient to appointments. The PN will incorporate 1) screening for a history of severe mental illness and risk factors for disruptions in cancer care at cancer diagnosis, 2) screening and monitoring for distress, and 3) streamlining referral/early consultation of social work, psychiatry, and palliative care. The PN will be a patient liaison with oncology and primary care team ensuring open lines of communication between the two teams. The PN will also be involved in other activities that are related to this program, as needed. What you will do Contact patients over the phone prior to first appointment to introduce and provide consistent contact calls to assure patients’ comfort Work with patients, providers, and TH Cancer Center care team to set goals for patient’s care and document.
