Job Description

The Nurse Practitioner examines and treats patients independently and in autonomous collaboration with other health care professionals following established standards and practices. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. Relies on education, experience, professional training and judgement to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision under the direction of the collaborating physician, as appropriate. May lead and direct the work of others within scope of practice.

Patient Care

Patient assessment and teaching and learning needs are accurately documented, detailed nursing history is obtained, and patients at physiologic and or psychological risk are identified as evidenced by physical assessments of patients that identify priority care needs.
Diagnosis is appropriately formulated with pertinent therapeutic and diagnostic procedures ordered at all times as evidenced by necessary procedures that are performed to implement plan of care based on care standards and, or protocols.
Treatment plan is implemented with consults and referrals with interdisciplinary team regarding patient assessment findings conducted as appropriate at all times.
Plan of care is reassessed, revised, and modified based on physiological and psychosocial findings as evidenced by documentation in the treatment plan. Patient plan of care is reviewed with interdisciplinary team at all times.
Tests and consults are ordered as required by plan of care, reviews test results 100% of the time. Plan of care is discussed with patient and family at all times as evidenced by documentation.
Education is provided to patients based upon identified risk factors as evidenced by patient education programs that are developed, revised, and evaluated based on the effectiveness of learning.

Partnership And Collaboration

All patient care activities are accurately documented as per documentation standards as evidenced by documentation of age appropriate, health education and community resource utilization being noted in records. Outcomes of documentation are evaluated and contributes to performance improvement initiatives through medical record review.
Multidisciplinary teams are utilized appropriately as evidenced by collaboration with the interdisciplinary team and incorporation of the expertise of the interdisciplinary team to achieve patient outcomes 100% of the time.
Students are precepted in accordance to specified guidelines.
Patient care activities are efficiently coordinated, and communication facilitated to maximize patient outcomes at all times and ensure continuity of care.
As a primary resource to patient care team, identifies, provides, develops, and maintains learning resources for patient care team as requested.

Quality

Patient documentation is accurate and legible based on focused chart reviews 100% of the time.
Safe therapeutic care is provided in holistic and systematic way to facilitate patients’ achievement of desired outcomes at all times.
Trends in patient populations are recognized, alternatives to care are explored and assistance is provided in the revision of standards, guidelines and or protocols as needed.
Participation in clinical research is provided and current nursing research literature is reviewed as evidenced by demonstrated application to clinical practice.
Practices that contribute to variance are analyzed, conclusions are drawn from aggregate data, resources to assist in variance analysis are identified and assistance with drawing conclusions from aggregate data is provided as evidenced by improved performance evidenced by outcomes.
Assistance is provided in the development and revision of standards for patient populations incorporating a interdisciplinary approach and utilizing best practices and research findings.

Job Description Under the supervision of the UCSF Department of Surgery & Department of Medicine and guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique to inpatient or ambulatory patient services (for those APPs providing services both in the inpatient and ambulatory environments) this Nurse Practitioner (NP) position is assigned to the Berkeley Cancer Outpatient Practice and Infusion Center. This APP position may perform delegated functions as appropriate to their “scope of practice”. In general, the “scope of practice” includes the management of chronic stable medical conditions, specialty protocols (e.g. chemotherapy), routine care, acute or critical care, and medical and surgical procedures. This position will provide outpatient care in approved age groups at UCSF Health settings. The APP has responsibility for the quality of standards of care for a specified patient population and evidences self-direction and accountability in the development of this role within the all UCSF medical center locations. UCSF Health requires all Advanced Practice Providers (APP) to be credentialed through its Medical Staff Services Office (MSSO) in order to practice and function in their clinical role. This applies to both adult and pediatric APPs in the inpatient and outpatient clinical settings at all UCSF Health sites (Parnassus, Mt. Zion, Satellite Clinics, et al). The Orthopedic Access Advanced Practice Provider (APP) will independently evaluate and manage patients with a broad spectrum of musculoskeletal conditions, with collaborative support from our subspecialty orthopedic teams. The primary objective of this clinic is to enhance timely access to orthopedic care. While the clinic will serve patients with a variety of musculoskeletal concerns, those with acute injuries will generally be directed to the Orthopedic Walk-In Clinic. For non-surgical cases, the APP will manage the patient’s care independently at our Prairie Meadow Drive location.. If surgical intervention is indicated, the APP will coordinate referral to the appropriate surgical team within the department. The role may also include performing in-office procedures such as joint injections, depending on the APP’s experience and clinical assessment. There may also be some cross coverage expected in the Orthopedic Injury Walk-In Clinic (North Liberty Campus). There the APP would see musculoskeletal injuries and pain that have occurred within 7 days. This occasionally requires splinting and casting. We provide training for this. This role requires someone who is organized, motivated, and very efficient. Schedule includes an evening shift every Thursday (12p-8pm) and every other Saturday (8a-2p). Position Responsibilities Ortho Access Clinic Perform evaluation and management for a variety of orthopedic conditions. Essential Functions In collaboration with physician(s), assesses patient, synthesizes and analyzes data, orders diagnostic tests and therapeutic treatments, and evaluates clinical outcomes. * Completes an accurate assessment (history and physical examination) of the pediatric patient. * Syntheses and analyses data obtained through assessment to develop a comprehensive plan of care that is individualized and reflects patient care needs and includes interventions and treatments to attain expected outcomes. * Implements plan by writing appropriate medical orders. * Uses jointly approved protocols appropriately, if required. * Evaluates and revises plan as indicated by changes in patient condition or needs. * Communicates medical plan to caregivers, staff members and physicians. * Demonstrates proper use of laboratory and x-ray to assist in diagnosis and interprets results correctly. * Demonstrates understanding of caregiver dynamics involved in the ill pediatric patient. * Utilizes the nursing process to meet the development and psychosocial needs of the pediatric patient/caregiver. * Assesses the educational needs of patients/caregivers and develops/implements the educational plan in collaboration with other healthcare professionals. * Prescribes controlled substances related to the patient’s health conditions within the scope of practice of the advanced practice registered nurse and according to state statutes. * Responds appropriately in crisis situations or life-threatening emergencies. Uses sound medical/clinical judgment during care for the pediatric patient. * Consults/collaborates with physicians as indicated. * Uses jointly approved protocols to guide decision making, if required. * Stays within Scope of Practice. * Documents assessments, plan of care, interventions and patient response to treatment. Assumes leadership role to establish and monitor standards of practice to improve patient care including recommending ways to improve clinical practice, participation in related educational activities, utilizes and/or participates in research, participation in professional organizations and performance improvement teams, and incorporation of new clinical information into practice. * Serves as a role model for staff by facilitating, supporting, mentoring, taking initiative, and practicing proactive problem solving. * Serves as a reliable and visible resource for staff. * Conducts or utilizes research and new clinical information to recommend ways to improve clinical practice. * Participates in educational activities and networking related to practice. * Seeks experiences and learning opportunities related to practice. * Participates in professional organizations. * Serves on Performance Improvement teams as needed. * Assures that clinical practice meets national standards of pediatric healthcare. * Coordinates project or program development and effective implementation Prescribes therapeutic measures and medications according to the State of Nebraska APRN/NP Statutes, as approved in his/her individualized Scope of Service, according to organization's policy. * Dispenses, incident to practice only, sample medications, which are provided by the manufacturer and are provided at no charge to the patient. * Prescribes therapeutic measures and medications, except controlled substances listed as Schedule II, related to health conditions within scope of practice. * Directs other team members in the delivery of patient care as determined by the needs of the patient.
