A Brief Overview Prescribes, delegates and coordinates professional nursing care to pediatric patients through the use of nursing process and in accordance with hospital policy/procedure and Clinical practice guidelines. Essential Functions Conducts holistic age-specific patient/family assessments upon visit according to defined standards. Ongoing patient/family Assessments/reassessments are completed according to the appropriate CPG or protocols and unit standard of care. Collects and documents data related to patient’s physical, mental, developmental, emotional and social condition. Demonstrates age-specific competency in assessment of patients by the ability to obtain and interpret information in terms of the patients’ needs, knowledge of growth and development, and understanding the range of treatment needed by these patients. Assesses family expectations for, and involvement in the assessment and continuous care or treatment of the patient. Able to meet families’ expectations during their visit to the clinic. Communicates plan of care to the patient/family so they can understand. Able to identify/assess the need for MD involvement or a potential change in plan of care. Ongoing patient/family assessments are completed according to the appropriate CPG, protocol and unit standard of care. Documentation and evaluation of nursing actions Chart audits conducted meets JCAHO and organizational expectations. Must meet overall goal of 95% (FER, Clinic problem list, signatures, pain assessment etc). Facilitates timely communication to/from patient/family and other healthcare disciplines; answers patient care related questions, requests etc. Phone calls are returned in the same business day received. Reviews patient charts as assigned to facilitate coordination of care by assuring completion of tests, consultations and procedures, interdisciplinary follow-up and referrals as needed for the clinic visit. Follows up on tests, lab results, thoroughly and within established time frames. Lab tests are followed up on within 48 hours after test results obtained. Considers cultural, cognitive, emotional, and social influences on the patient/families ability and readiness to learn. All teaching is documented on the FER to include understanding of topics taught. Provider Support Gathers patient information in a timely, concise, thorough manner and conveys this to the MD. Assists MD in clinic to maintain a good workflow and meet expectations of customers in relation to being on time. Demonstrates effective delegation techniques. Performs special tasks, procedures within the scope of practice for an RN that is unique to Ambulatory Care clinics i.e. (database entry, scheduling of special procedures, as well as assisting in special procedures, inpatient role, case management, and education/teaching). Completes with expertise, timely, organized and with detail. Communicates effectively through written documentation or verbally to appropriate individuals. Coordinates patient care to ensure continuity of care This role will require weekend call from 8am- 4:30pm for inpatient diabetes education. The frequency of On Call assignments are based on Endo Clinic RN FTE. Essential Functions Assumes responsibility for the orientation of new employees to the unit. Creates an orientation program for each role in the unit. Collaborates with unit manager to counsel/coach new employees during orientation to assist the orientee’s progress towards gaining competency in unit standards/skills. Supports the preceptor role by: Collaborating with CN, department leaders and preceptor to select appropriate assignments and learning experiences for new staff. Fostering the use of adult learning strategies and critical thinking with preceptors by providing ideas, skill stations, different teaching styles, and related activities to help the preceptor be a more effective teacher. Assumes responsibility for the coordination of staff education processes through the development, implementation, and evaluation of education materials/systems. Collects and documents data related to patient’s response to procedure and changes in condition according to hospital policy. Documentation of data is entered in appropriate medical records. Demonstrates age-specific competency in assessment of patients by the ability to obtain and interpret information in terms of the patient’s needs, knowledge of growth and development, and understanding of the treatment needed. Circulates in procedures following AORN guidelines/standards. Assists at bedside in preparing patient for each procedure. Applies critical thinking during catheterization procedures in the delivery of direct patient care and in analyzing response to medications and treatment, prioritizing intervention based on continual assessment. Utilizes appropriate telephone etiquette. Develops individualized patient/family plan of care using CPG/ND’s, and practice guidelines/protocols to meet identified needs. (Plan of Care includes current and anticipated actions focused on care, education, discharge needs.) Utilizes assessment data from the healthcare team to individualize appropriate goals and actions for the patient, including collaboration with the family and the healthcare team. Utilizes individualized assessment data to develop the patient/family’s plan of care. Formulates goals and outcomes based on assessment data. Revises plan of care based on patient’s condition, interdisciplinary input, and family needs. Demonstrates ability to prioritize patient interventions based on assessments/reassessments Implements and evaluates nursing actions in accordance with the individualized patient/family plan of care and patient condition/needs. Oversees that the nursing and interdisciplinary plan of care/goals are coordinated, implemented, evaluated, and revised as needed. Identifies the need for interdisciplinary involvement/outside resources and follows through. Uses appropriate resources (CNS/Educator, Director, MD, etc) to ensure that clinical outcomes are achieved. Incorporates critical thinking into the delivery of clinical skills and in analyzing patient response to orders, tests, labs, medications, and treatments. Demonstrates clinical competence in administering interventions according to identified standards and age specific needs. Provides and/or coordinates with other disciplines patient/family education according to identified care needs, considering assessment of patient/family’s readiness to learn Cath lab: Coordinates with referring physician and other operating physician, assessing patient/family care issues in scheduling procedures Cath lab: Accompanies patients/family to room to help facilitate patient flow. Documentation reflects accurate assessment/evaluation, modification, and patient’s response to therapy, changes in condition, and therapeutic interventions/procedures. Entries in the medical record follow hospital policy (i.e., date, time, use of appropriate abbreviations, and legibility). Documentation is completed within established timeframes including, but not limited to: Admission Assessment form, daily Patient Flowsheet, Peripheral IV Flowsheet, Family Education Record, and Interdisciplinary Goals. Reviews and updates computerized careplan to accurately reflect the plan of care. Completes computerized documentation within established timeframes, including, but not limited to: Medication Administration Record, Allergies, Patient Information, etc. Completes and verifies accurate order entry in the computerized careplan. Ensures that discharge instructions clearly state patient care needs after discharge. Regular attendance at work is an essential function of the job. Education Qualifications Associate's Degree from an accredited school of nursing Required or Bachelor's Degree from an accredited school of nursing Required Experience Qualifications Minimum 1 year of RN experience Required and Experience with pediatric patients Preferred and Previous experience with EPIC Preferred Skills And Abilities Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing skills.
