POSITION SUMMARY:
To be fully engaged in providing Quality/No Harm, Customer Experience and Stewardship by performing direct patient care, under the supervision of an RN, utilizing nursing practice/care standards and elements of good service for effective delivery of home nursing services.
PRIMARY ACCOUNTABILITIES:
Quality/No Harm:
- Collaborates with the Registered Nurse in utilizing the nursing process to provide responsible patient care through:
- Participate with the Registered Nurse in assessing the patient's health status per established standards.
- Documents and reports deviations from the patient's usual or expected responses.
- Reinforces patient and family teaching instituted by the Registered Nurse.
- Accepts responsibility for personal and professional accountability and development, including maintenance of all required personnel credentials.
- Performs procedures and treatments with skill and efficiency.
- Utilizes a standard nursing care plan to select desired patient outcomes. Coordinates the nursing care plan with the medical plan of treatment.
- Collaborates with the Registered Nurse to revise the nursing care plan based on evaluation of the patient's responses to nursing care and progress toward goal achievement.
Customer Experience:
- Communicates effectively with RN, patients, families, staff and physicians.
Stewardship:
- Other duties as assigned
Work Experience
QUALIFICATIONS REQUIRED:
- Current Licensed Practical Nurse in the state of Florida
- Current American Heart Association BLS CPR card.
- A current American Safety and Health Institute (ASHI) BLS CPR card for Healthcare Providers will be accepted upon hire. However, associate must obtain American Heart Association BLS CPR card within 90-days.
- Exception for HF Private Duty Associates: A current Red Cross BLS CPR card will be accepted upon hire. However, upon the next renewal period the associate will be required to renew their CPR certification with an approved AHA provider. Per policy No.: HR 1.14.
- Prefer one year home care and/or facility-based experience
- Successful completion of Level II background screening (fingerprinting) after August 1, 2010.
- Current valid Driver License or ID card.
- Exposure to biohazardous waste, chemical substances and unknown diseases.
- Care is provided in a variety of home environments.
PHYSICAL DEMANDS:
Ability to move in crowded conditions.
- Ability to operate vehicle in variable conditions.
- Visual acuity and hand-eye coordination to perform clinical tasks.
- Ability to lift, turn, pull and reach for heavy objects.
- Ability to ambulate patients up to 20 times daily.
- Ability to lift up to 40 pounds, unassisted, up to six times daily.
- Ability to speak and understand the English language.
- Risk of exposure to infectious disease.
- Ability to stand, walk, bend and stoop for 90% of assigned shift.
MENTAL DEMANDS:
- Ability to interact with other professionals, families and patients.
- Ability to work in stressful environments/situations.
- Ability to quickly decide concerning appropriate therapy and patient care.
- Ability to respond appropriately to life threatening situations.
- Ability to assess, interpret data and educate patients and families of various ages and levels of understanding.
