Cardiac Tele PCU Department Highlights

Cardiac Tele PCU is a 24 bed post interventional unit mixed with medical telemetry providing comprehensive post interventional cardiac care to patients from admission to discharge.
We specialize in the care of cardiovascular patients which includes Heart Failure, AMI and Arrhythmia patients, pre and post interventional patients (Cath, Ablations, & Pacemakers).
Self-scheduling which helps with the work/life balance as well.
AMI program has achieved the trifecta of awards for the 2nd year in a row from the American Heart Association and National Cardiovascular Data

Here’s What You Need

Diploma Nurse required or Associate degree from an accredited Nursing program required.
BSN preferred
One-year experience as a clinical nurse, or completion of an RN residency program.
PCU, Step-down Intermediate, Cardiac Tele highly preferred.
RN must practice within the regulations of the Texas Nurse Practice Act, serve as a role model, intra/interdisciplinary collaborator, and coordinator for quality patient care.

Field Care Manager, LTSS (RN) - Local Travel Required

JOB DESCRIPTION Opportunity for TX RN to join Molina as a Care Manager working with our Medicaid members in the northern part of the Fort Worth, TX service delivery area. Communities serviced will be North Richland Hill, Hurst, Richland Hills, and North Arlington. Responsibilities include completing in-home, face-to-face assessments with our waiver Medicaid members there. Preference will be given to RNs with this type of experience having worked for an MCO organization like Molina. Hours are Monday – Friday, 8 AM – 5 PM CST and mileage is reimbursed as part of our benefit package. Need to be bilingual. Solid experience with Microsoft Office Suite is necessary, especially with Outlook, Excel, and Teams as well as being confident in moving between different programs to complete the necessary forms and documentation. Job Summary Provides support for care management/care coordination long-term services and supports (LTSS)-specific activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care.
