About the Wound Nurse (RN) Role

As a Wound Nurse (RN) at Aspire Senior Living Moberly, you will play a vital role in providing exceptional wound care to our residents. Wound Nurse (RN) will be responsible for assessing, planning, implementing, and evaluating individualized wound care plans, as well as collaborating with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities

• Conduct thorough wound assessments and develop individualized wound care plans
• Implement and maintain wound care plans, including wound dressings, medication management, and other treatments
• Monitor and document wound progress, and adjust treatment plans as needed
• Collaborate with the interdisciplinary team, including physicians, physical therapists, occupational therapists, and other healthcare professionals
• Educate residents, families, and staff on wound care and prevention strategies
• Participate in quality improvement initiatives to facilitate best practices and improve patient outcomes

Requirements

• Active Registered Nurse (RN) licensure in the state of Missouri
• Experience in Infection prevention, wound or related skills preferred
• Strong knowledge of wound care principles, practices, and regulations
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced, dynamic environment

Overview Upon consultation, the Wound and Ostomy Specialist assesses, plans implements and evaluates wound and ostomy care on inpatients throughout the hospital. Provides direct patient care and education to selected patients. Serves as a resource and educator to the staff nurse and physician. Is an integral member of the treatment team and is responsible for the case management of specific patients an in working autonomously and in collaboration with the physicians. The Wound Care Specialist will serve as a resource and educator to physicians and patients when requested by the MD and as staffing permits in the outpatient clinic areas. Responsibilities Wound Assessment and Treatment: Conduct comprehensive assessment of wounds, including pressure injuries, surgical wounds, burns, diabetic ulcers, and other acute or chronic wounds Identify the type, severity and stage of wounds and document findings accurately in the electronic health record. Develop and implement individualized wound care plans based on evidence-based practice. Wound Care Management: Perform wound care and application of appropriate dressings or wound care products. Monitor wound healing progress and adjust POC accordingly. Apply, monitor and manage negative pressure wound therapy. Ostomy Assessment and Management:Conduct assessment of new and existing ostomies and educate patient, caregiver and staff on appliance and management of. Patient and Family Education: Educate patients and caregivers on proper wound care techniques, prevention of infection and lifestyle adjustments to promote wound healing. Provide guidance on nutrition and hydration to support wound healing and refer to nutrition services when necessary. Collaboration with interdisciplinary team: Collaborate with providers and other allied healthcare professionals to optimize patient outcomes. Provide recommendations on wound management to the healthcare team and participate in discharge planning. Documentation and Reporting: Maintain accurate, detailed and timely documentations of wound assessments, treatments and patient responses. Maintain and report on hospital acquired pressure injuries. This includes drilldowns and preventive measure/actions plans.
