Responsibilities:

Develop and implement wellness initiatives.
Create and manage wellness programs.
Conduct health assessments and identify health risks.
Collaborate with health professionals to develop strategies for improving the health of employees.
Provide education, coaching, and support to employees on health and wellbeing topics.
Monitor and evaluate the effectiveness of wellness programs.
Provide advice and guidance on health and wellness topics.
Develop outreach and promotional activities to increase awareness of wellness programs.
Oversee facility maintenance and upkeep.

Qualifications:

At least 3-5 years experience as Wellness Director in an Assisted Living Community.
Must have RCFE experience and active LVN license.
Excellent communication and interpersonal skills.
Ability to develop, implement, and evaluate wellness programs.
A good understanding of health and well-being issues.
Knowledge of health promotion strategies.
Ability to work collaboratively with multiple stakeholders.

Primary Duties and Responsibilities Sets up exam room/procedure rooms appropriately based on patient visit needs Performs initial nursing intake and prepares patients for examination. Description Kindred Hospital Brea helps patients return to the lives they love. At our 48-bed hospital, our physician-led care teams provide critically ill patients with the specialized acute care and rehabilitation they need, for the time they need it - empowering them to take the next step in their recovery journey. We also offer a 4-bed ICU and three negative pressure rooms to accommodate particularly serious conditions. We are located less than a mile north of Brea Junior High School and conveniently surrounded by restaurants and shops. Job Summary Under direct supervision of Registered Nurse, provides nursing care within the scope of licensure for assigned patients according to established nursing standards and hospital policies and procedures. The LPN/LVN Charge Nurse manages a designated group of nursing employees on a nursing unit and assures smooth operations of the unit for a designated shift. Could encompass one or more units depending upon size and clinical programs. The scope of work may be modified by State specific rules under the Nurse Practice Act. Essential Functions Assesses, plans, coordinates and evaluates patients’ care along with members of the interdisciplinary team. Documents the patient’s condition and nursing needs accurately and in a timely manner in the patient’s EHR. Reports pertinent observations and reactions regarding patients promptly to clinical leader and Physician as necessary. Receives and records physicians’ orders.
