Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

  • Admissions, Discharge and Transfers of residents

  • Medication requisitions with pharmacy and physicians

  • Medication administration

  • Following infection control, emergencies, and other center policies

  • Collaborating with resident’s interdisciplinary team to provide the best outcomes

  • Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy

  • Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.

  • Delivers nursing care to patients/residents requiring long-term or rehabilitative care.

  • Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.

  • According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.

  • In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.

  • Maintains knowledge of necessary documentation requirements.

  • Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.

  • Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

  • LPN (Licensed Practical Nurse) licensed by the State

  • CPR Certification required

  • Two Years prior nursing experience preferred

Update the electronic health record (EHR) system with all relevant information promptly. Ensure an active transition of the neighbors entering the facility from another care setting. Resident and Family Support: Welcome neighbors and their families upon admission/re-admission, providing a thorough orientation to the facility and its services. Act as a liaison between residents, families, and the healthcare team, addressing questions and concerns with empathy and professionalism. Provide education to residents and families about care plans, medications, and facility policies. Collaborate with unit-based staff to ensure room readiness Collaboration and Communication: Work closely with the admissions team, nursing staff, and other departments to coordinate the arrival and placement of new/returning neighbors. Collaborate with external healthcare providers, hospitals, and referral sources to ensure continuity of care. Quality Assurance: Monitor and evaluate the admissions process, identifying areas for improvement and implementing solutions. Ensure that neighbors’ initial care plans align with their specific needs and that care delivery meets the facility’s standards. Other Duties: Act in a nursing supervisor capacity when needed.
