Essential Functions
This Position Identifies bottlenecks in the system. Works with physicians and LifeLine to appropriately triage patients for discharge and transfer and mobilize resources to accomplish that in a timely, efficient fashion. Facilitates safe, timely, and efficient patient discharges by proactively coordinating care among the interdisciplinary healthcare team throughout the system, working with on-site discharge teams. Identifies and resolve barriers to discharge, streamlining communication, and ensuring that clinical, educational, and logistical needs are met to support optimal patient flow. Expedites discharge readiness while maintaining quality and patient safety, the to reduce length of stay, prevent discharge delays, improve patient and family satisfaction, and support continuity of care across settings.

Full time day shift 6A-2P Mon-Fri.

QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES
• Bachelor's Degree or equivalent years of experience is required.
• Requires 6-8 years of relevant experience.
• Requires a valid and current Registered Nurse License.
• Basic Life Support certification through the American Heart Association (AHA) is required.
• Requires proficiency in Cerner and TeleTracking.
• Requires basic proficiency in reviewing and analyzing data.
• Case Management Certification or Care Transitions Certification preferred.
