HART Patient Care Coordinator RN

Job Summary and Responsibilities

As a HART (Heart and Rhythm Team) Patient Care Coordinator RN, you will provide expert nursing leadership and meticulous coordination, ensuring seamless care for complex cardiac arrhythmia patients.

Every day you will expertly manage patient pathways, educate patients and families on treatment plans, schedule advanced cardiac procedures, and meticulously collaborate with multidisciplinary teams to optimize outcomes for those with heart rhythm disorders.

To be successful, you will demonstrate outstanding clinical knowledge in cardiology, strong organizational skills, and a proactive, empathetic demeanor, contributing significantly to improved patient experience and specialized cardiac care.

  • Interviews and evaluates patients for hospice eligibility in accordance with Medicare criteria.
  • Explains hospice services and eligibility criteria, and elicits clinical information from patient and/or family regarding health status and readiness for hospice care.
  • Prepares patient assessments using established admission criteria to determine appropriateness for Hospice services.
  • Admits patients to hospice services in accordance with established procedures.
  • Communicates patient’s condition and needs to the interdisciplinary group for further care plan management.
  • Informs referral source of patient’s admission status in a timely manner; obtains and documents information regarding advanced directives, and obtains signed DNR as appropriate; ensures appropriate documentation is completed in accordance with established standards.
Job Requirements

Required

  • Graduation from an accredited school of nursing and two years of related nursing work experience in an acute clinical care setting within the last five years that demonstrates attainment of the requisite job knowledge skills/abilities, upon hire and
  • Work experience in one of the following clinical specialty areas required: Oncology, Med/Surg, Nutrition Support, Pediatrics, IV Therapy, Gerontology, Home Health. and
  • Registered Nurse: WA, upon hire and
  • Basic Life Support - CPR, upon hire


Preferred

  • Bachelors Of Science Nursing, upon hire
