Responsibilities

The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/ or palliative care to patients and families

Performs prescribed medical treatments, including patient examination, diagnoses, medication prescription, pain management and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients and caregivers

Performs comprehensive patient examinations, evaluations, assessments and diagnosis

•

Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to control and manage patient symptoms and needs

•

Makes adjustments to the already established plan of care and appropriate services, provides appropriate follow- up to all patients

•

Demonstrates flexibility in scheduling, and readily accepts new and emergency assignments as requested

•

Performs timely face to face assessments with patients

•

Demonstrates positive interpersonal relation in communicating with physicians and co-workers to promote quality patient care

•

Completes all documentation in an accurate, legible and timely manner

•

Provides quality patient care consistent with standards of nursing practices

•

Complies with hospice policies and procedures, laws and regulations and standards of practice

Job Description The University of California, San Francisco Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) for a night-shift position to care for patients with advanced heart failure, mechanical circulatory support (MCS), heart transplantation, and other complex cardiovascular conditions. The APP will join a highly experienced multidisciplinary team of physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers who provide comprehensive care for advanced heart failure patients across the continuum of care. UCSF’s Advanced Heart Failure and MCS programs are nationally recognized for delivering cutting-edge therapies and support in both inpatient and outpatient settings, including a wide range of temporary and durable mechanical circulatory support options. Serving as a primary provider in the hospital setting, the APP will collaborate closely with Advanced Heart Failure Cardiologists, Cardiothoracic Surgery and Intensive Care teams, as well as nursing, case management, social work, and other multidisciplinary partners to evaluate, treat, and manage this highly complex patient population in both Intensive Care and Acute Care Units. Job Description Under the supervision of the UCSF Department of OB/GYN, REI guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique ambulatory patient services this Nurse Practitioner (NP) position is assigned to in the Center for Reproductive Health (CRH). This AHP position may perform delegated functions as appropriate to their "scope of practice". In general, the "scope of practice" includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per approved standardized procedures as identified by the list available on the Medical Staff Office website.
