 Hours: Friday, Saturday and Sunday night   (8pm to 0830am)

 

The RN-On Call Nurse is responsible for, but not limited to:

  1. Assumes responsibility for a patient visit or on call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process
  2. Provides hospice nursing care based on systematic assessment docusing on pain control and symptom management
  3. Initiates communication with attending physicians, other hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family
  4. Maintains regular communication with direct supervisor to review assignments
  5. Maintains up to date documentation at point of service so that problems, plans, actions and goals are accurately and clearly stated and charges are reflected as they occur in accordance with agency policy and Medicare regulation
  6. Administers medication in accordance with all federal and state laws and the policies of the Hospice along with LTC staff
  7. Maintains current patient records and documents in accordance with the policies of Ascend Hospice to ensure that problems, intervention, goals and outcomes are accurately and clearly states and charges are reflected as they occur
  8. Assess and collaborates with Medical Director to determine initial and on going eligibility of the hospice patient
  9. Completes an initial assessment of patient and family to determine home care needs
  10. Can anticipate end stage disease process and related pain and symptom management
  11. Regularly re-evaluates patient's nursing needs
  12. Uses assessment data to determine care plan needs
  13. Initiates appropriate preventive and palliative nursing procedures
  14. Counsels the patient and family in meeting end of life needs
  15. Performs a comprehensive death note when appropriate
  16. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition
  17. Communicates with other members of the IDT to coordinate the care plan
  18. Participates in On-call duties as defined by the on-call policy
  19. Ensures that arrangements for equipment and other necessary items and services are needed

Education:

Associate's Degree in Nursing

 

Professional Experience:

  • Must have a State Certified RN license
  • Experience in either Hospice or home health care, REQUIRED
  • Must have excellent documentation skills and the ability to work independently.
  • Strong communication skills.
  • Valid driver's license & reliable transportation.
  • Must be available to travel locally within assigned territory.

Job Summary: Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.
