Regional MDS Coordinator
As a Regional MDS Coordinator, you will play a vital role in ensuring the highest quality of care for our residents.
Qualifications
· 1 years of experience (education or training in a healthcare setting)
· Strong clinical background in a long-term care setting
· Excellent communication, organizational, and problem-solving skills
· Ability to work collaboratively as part of a healthcare team
· Familiarity with MDS processes and regulations (training provided for the right candidate)
Responsibilities
· As a Regional MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) process across multiple facilities within our region
· This includes ensuring accurate and timely completion of MDS assessments, coordinating with healthcare teams to address quality of care concerns, and maintaining compliance with all relevant regulations and standards
· Your strong clinical background and excellent communication skills will enable you to effectively collaborate with staff, physicians, and other healthcare professionals to achieve our goals
· Coordinate MDS assessments and updates, ensuring accuracy and compliance with regulations
· Collaborate with healthcare teams to identify and address quality of care concerns
· Develop and implement strategies to improve clinical outcomes and quality of care
· Maintain accurate and up-to-date records of MDS assessments and quality care initiatives
· Provide education and training to staff on MDS processes and clinical care standards
· Participate in quality improvement initiatives and contribute to the development of quality improvement projects
· Develop and Maintain Case Mix Index (CMI) for your region
· Work closely with finance team to ensure maximal reimbursement
