This is an onsite/ non-remote position.
As our MDS Coordinator, you will play a vital role in ensuring accurate assessments, regulatory compliance, and quality resident outcomes. This is an excellent opportunity for a detail-oriented nurse who enjoys working closely with an interdisciplinary team and making a meaningful impact every day.
Responsibilities:
Complete and coordinate MDS assessments in accordance with state and federal regulations.
Ensure accurate and timely completion of care plans and required documentation.
Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
Monitor clinical documentation to support appropriate reimbursement and regulatory compliance.
Participate in quality improvement initiatives and survey preparedness.
Educate and support staff regarding MDS processes and documentation requirements.
Qualifications:
Active LPN or RN license required. RN preferred.
Previous MDS Coordinator experience in a long-term care or skilled nursing setting required.
RAC-CT certification preferred.
Working knowledge of Medicare, Medicaid, the RAI process, and PDPM.
Strong assessment, organizational, and critical thinking skills.
Experience with PointClickCare (PCC) preferred.
Excellent communication and teamwork abilities.
