QUALIFICATIONS & ATTRIBUTES

Licensed Practical Nurse with current license in the state of employment.
Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred.
Home care experience is desired.
Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.
Ability to remain calm, have patience and be accommodating.
Compassionate and caring while working with patients.
Knowledgeable on nursing best practices.

MAJOR AREAS OF RESPONSIBILITY

Provides skilled nursing care in a patient’s home as ordered by the attending physician.
Performs care by following order sets and goals made by RN.
Observes and monitors patient conditions.
Notifies RNCM of any changes in patient condition.
Administers medication as prescribed by the physician.
Counsels patient and family on the disease/injury processes and how to manage.
Incorporates patient and family in development of plan of care.
Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
Listens to patient and family members.
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
Documents all aspects of treatment, assessments, and patient education.
Maintains active LPN license in state of employment.
Communicates with scheduler any changes outside the normal.
Participates in all on-call requirements and case conferences.
Attends in-service trainings and mandatory agency meetings.
Maintains a professional appearance as a representative of the company.
Understands and practices agency policies and procedures.
Other duties as assigned.

ESSENTIAL FUNCTIONS

Drives to patient’s primary location per scheduled visit.
Daily attendance at assigned visit locations.
Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines.
Completes and submits all required documentation within specified company requirements.
Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.
Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.

JOB RESPONSIBILITIES: Gathers data on patient's physical and emotional needs and provides prioritized school nursing care according to patient age, symptoms, and psychological needs before/during and after school when involved in school activities, under the supervision of a Registered Nurse. Monitors the health needs of the students and faculty and consults with Registered Nurse as needed. Monitors patients systematically to identify progress and trends that require interventions. Complete patient/family education and discharge plans identified in the patient's plan of care Planning: a. Collaborate with Staff RN/Case manager to identify the patient's present and potential problems from the assessment. b. Implement a teaching plan in collaboration with Staff RN/Case Manager. c. Prioritize problems according to impact on health status. d. Formulate with Staff RN/Case Manager the desired outcomes that are specific and measurable within a certain time frame and consistent with other health provider's expectations. e.
