Responsibilities
What You’ll Do
- Provide community-based nursing care coordination across CFCM, Shared Living (SLA), and BHDDH programs
- Conduct comprehensive health assessments and develop individualized nursing care plans
- Coordinate care across medical, behavioral health, specialty providers, hospitals, and community services
- Serve as a clinical resource to Shared Living Providers, offering guidance, training, and ongoing support
- Monitor chronic conditions, medications, treatment plans, and health outcomes
- Review hospital discharges, ER visits, and clinical recommendations to ensure timely follow-up
- Deliver education on health promotion, medication safety, infection control, and chronic disease management
- Ensure compliance with BHDDH, EOHHS, Medicaid, HIPAA, and agency standards
- Participate in interdisciplinary team meetings, quality reviews, and incident management processes
- Advocate for individuals served and promote person-centered, community-based care
Qualifications
What We’re Looking For
- Active Registered Nurse (RN) license (Rhode Island required)
- Bachelor’s degree in Nursing required
- 3–5 years of nursing experience, preferably in community health, IDD, behavioral health, or care coordination
- CPR/First Aid certification (or ability to obtain)
- Valid driver’s license (Class C or D)
