Registered Nurse (RN) – Crisis Respite / Intensive Case Management / Critical Time Intervention

About the Opportunity
Bond Health Staffing is seeking a compassionate and experienced Registered Nurse (RN) to support individuals with intellectual and developmental disabilities (IDD) and severe mental illness (SMI).
This role provides comprehensive nursing care across crisis respite, intensive case management, and community transition services.
The RN works closely with an interdisciplinary team to promote health, safety, independence, and successful community integration.
Schedule
• Monday – Friday
• 9:00 AM – 5:00 PM
Key Responsibilities
Participate in the intake process by screening individuals for program admission.
Conduct initial and ongoing nursing assessments.
Oversee self-medication administration and monitor therapeutic outcomes.
Provide direct nursing care including wound care, diabetes management, respiratory support, and chronic disease management.
Recognize early signs of medical or psychiatric crises and implement appropriate interventions.
Review medication administration records, vital signs, glucose logs, weights, and other clinical documentation.
Communicate significant medical concerns to providers and coordinate medication renewals and updates.
Maintain medication storage and disposal in compliance with applicable regulations.
Develop, monitor, and update individualized health goals.
Ensure infection control standards and universal precautions are followed.
Maintain necessary medical and first aid supplies.
Provide on-call phone consultation as needed.
Partner with case management teams to coordinate medical and behavioral health services.
Provide health education to individuals, families, and direct support professionals.
Coordinate hospital discharges and community reintegration services.
Arrange follow-up care with primary care providers, specialists, and community resources.
Serve as a clinical liaison between providers, families, and program staff.
Participate in interdisciplinary treatment planning and case conferences.
Train and mentor Direct Support Professionals (DSPs) on health-related procedures and emergency response.
Maintain accurate and timely clinical documentation.
Qualifications
Current unrestricted New York State Registered Nurse (RN) license.
Minimum of two years of nursing experience.
Experience working with individuals with IDD, behavioral health, crisis care, or community health is preferred.
Knowledge of OPWDD, OMH, and DOH regulations is preferred.
Strong assessment, crisis intervention, care coordination, communication, and documentation skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Valid driver's license preferred.
