Job Duties:

  • Provides patients' personal hygiene by assisting with baths, shampoos, and shaves; assisting with travel to the bathroom; helping with showers and baths.
  • Provides for activities of daily living by assisting with serving meals, feeding patients as necessary; ambulating, turning, and positioning patients; providing fresh water and nourishment between meals. checking vital signs and weight; recording intake and output information.
  • Provides patient comfort by utilizing resources and materials; transporting patients; answering patients' call lights and requests; reporting observations of the patient to nursing supervisor.
  • Documents actions by completing forms, reports, logs, and records. Maintains work operations by following policies and procedures.

Promotes and supports the greatest possible degree of independence for residents. Advises supervising charge nurse of resident’s status at beginning and end of shift. Contributes knowledge of resident’s conditions/needs to resident care plans. Assist in the implementation of care plans as directed within the parameters of established policy and procedure. Observes and reports on residents’ conditions, and documents observations as required. This job is primarily from 8 - 5 Monday - Friday, however there are times when you must be able to participate in after 5 p.m. hospital admits or re-admits. There is also an occasional weekend admission that requires the transportation person to assist with. The van driver does assist with resident care on days/times when there are no appointments/transports scheduled.
