Hi there! I wanted to start this job posting by personally introducing myself:  My name is Carly, and I am the owner of SYNERGY HomeCare of St. Charles.

• Companionship and personal care

• Bathing, dressing, grooming, and toileting

• Hospital - to - Home Transition Care (higher-acuity cases) 

• Hospice (higher-acuity cases) 

• Dementia and memory care support (higher-acuity cases) 

• Transfers, walking assistance, and fall prevention

• Light housekeeping, meals, and medication reminders

• Errands or transportation, depending on the client
