Job Role Summary This position is responsible for developing and implementing educational opportunities for CBC Public Health Nurses, other Community-Based Care (CBC) staff, and the public. The Public Health Nurse (PHN) Educator will help plan and teach new PHN orientation sessions.  In addition, the PHN Educator will provide comprehensive nursing services that promote the health and well-being of residents in Marion County by providing preventive, therapeutic, and educative follow-up to clients in homes, schools, workplace and/or community sites.  The PHN Educator may work with individuals and families to promote health and wellness through assessments, counseling and prevention services.  Representing CBC, the PHN Educator will work with many different departments at MCPHD. Working with a diverse population with different cultural backgrounds and varied languages, the PHN Educator must be flexible and adaptive in providing continuity of care.  Associated Job Duties Responsibility 1: 60% Identify educational needs within CBC and/or within nursing staff in MCPHD.   Develop, plan and implement educational sessions for CBC Public Health Nurses and Community Health Workers.   Plan and implement CBC orientation for new employees with CBC Administrator and CBC Coordinators Work with CBC Administrator and Coordinators to plan, implement, and complete both annual training and orientation for the CBC School Screeners.  Collaborate with CBC Administrator and Coordinators to develop, review and update CBC Policies and Procedures. Will also be responsible for training the PHNs on new policies and procedures. Motivated and dedicated teacher, up-to-date with the latest nursing research and clinical practices. Utilize evidence-based practice and research as indicated. Must possess strong computer skills.  Be able to manage and utilize many different programs and systems at one time. Well-educated and knowledgeable and possess strong clinical experience and excellent communication skills.    Responsibility 2: 15% Interview client to assess immediate needs of the client and/or family.  Health history is recorded; nursing care plans are developed; referrals are made to appropriate agencies and the continuity of care for the client/family is maintained through home visitation and follow-up.  Provides health education and counseling to individuals and groups.  Participates as a team member to provide disease prevention guidelines and surveillance of health practices with regard to physiological, psychological and social processes. Provides comprehensive case management and coordinates services provided by a multidisciplinary approach using MCPHD resources and community resources.  Initiates appropriate referrals to agencies/organizations based on needed client services/supplies. Collaborates and coordinates nursing services with other community agencies, organizations, health care providers and social services. Follows MCPHD/CBC Standards of Nursing Care. Documentation must be clear and concise and recorded in a timely manner.  Electronic medical records require the use of typing and computer skills. Must be able to communicate effectively in both verbal and written form. Must be able to accept direction and work with other members of the team. Must be able to maintain client confidentiality and client records are properly stored.  Follows HIPAA requirements.   Responsibility 3:  10% Performs appropriate diagnostic procedures, treatment regimens, and administers injections and medications according to physician orders and/or MCPHD protocols and policies.  Delivers Nurse of the Day (NOD) services as assigned.  NOD is a walk-in service that is provided to assist the community with services such as blood pressure screening, counseling and education, referrals for needed services, infant, child and adult vaccines, TB testing and medication follow-up. Administers vaccines in immunization clinic.  Vaccines are administered safely and accurately. Provides services through other clinic assignments, i.e., Refugee, etc. Obtains blood pressure, blood draws for various testing and collects other data and develops information from the health history for follow-up.  Performs a variety of related procedures.   Responsibility 4:  10% Collaborates with other MCPHD departments and outside agencies to provide necessary services in the community.  Examples would be Heart Alive, Communicable Disease outbreaks; Bioterrorism/Biological warfare programs, School services, Health Fairs, etc.  Qualifications Bachelor’s degree in nursing from a NLN accredited college 3 or more years of previous nursing experience preferred, with preference given to those with previous education and/or public health experience Must possess strong computer skills, including typing skills of a minimum of 30 words per minute, familiarity with Microsoft Office (Word, Excel, PowerPoint) Current Indiana RN license CPR certified Requires a valid Indiana Driver’s license Requires access to own vehicle and current automobile insurance Licenses/Certifications Required Current Indiana RN license CPR certified Requires a valid Indiana Driver’s license Requires access to own vehicle and current automobile insurance Knowledge, Skills & Abilities Ability to function independently; set priorities; flexibility Must have excellent communication and interpersonal skills Work effectively in stressful situations Requires excellent organizational skills Considerable decision-making, good judgment, assessment ability, prioritization skills. BEHAVIORAL HEALTH RN CASE MANAGER

JOB RESPONSIBILITIES: Assess and analyze the patient’s record according to pre-established criteria for severity of illness and intensity of services provided. Compare data from the patient record to criteria and collaborate with the healthcare team to assure appropriate utilization of services. Serve as consultant to the provider, assuring proper placement within the continuum of care. Complete reviews accurately and timely, using Interqual criteria. Monitor observation stays, assure admission to inpatient as soon as inpatient criteria has been met, or ensure discharge from observation status as soon as possible. Screen for Medical Staff quality issues using criteria set by Medical Staff. Reviews insurance verification information in Electronic Medical Record (EMR) and determines what medical information is required for certification of stay/services. Takes action based on insurance companies' requirements to ensure that appropriate information is submitted and time frames are met for certification and to comply with reimbursement standards. Works with supervisor to obtain certification from insurance companies for patient stays. This involves obtaining clinical information from CERME and faxing required information to insurance companies. Obtains insurance information and approvals for discharges. This would include contacting insurance companies for information needed to facilitate discharge. Assess the health status of assigned patients by collecting and analyzing patient data and evaluate appropriateness of nursing, medical and ancillary care. Evaluate potential for safe discharge to home, identifying limitations and need for referral services. Thoroughly and accurately document the discharge plan ensuring regulatory compliance. Tracks faxes sent to insurance companies to ensure certification information is received. Reviews the EMR when further information is needed by insurance companies and follows up to make sure that information is submitted in a timely fashion. Tracks patients that are discharged and faxes discharge information when appropriate. Obtains and tracks certification on patients that have been discharged. Assess and evaluate overall patient plan of care/care pathways in collaboration with attending provider and team members and intervene as appropriate on a concurrent basis to resolve potential patient-specific avoidable days. Maintain knowledge of discharge agencies/resources available to meet the patient’s needs. Monitor use of resources such as time, people, and products to achieve patient outcomes and decrease length of stay. Counsel patients/families on “Prudent Lay Person” list from Medicaid and appropriate utilization of Emergency Services. Counsel patients/families on financial options available for payment of medical services (i.e., referral to Claim Aid). Serve as liaison with third party payer to obtain authorization/certification for the hospitalization. Participates with and assists nursing staff in performance improvement initiatives as identified by the department, Utilization Review Committee, or other JMH-directed activities.
