About the Role

As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will build strong relationships with the facility, maintain a patient-centered approach, and collaborate with our clinical team to provide exceptional care.

Responsibilities

  • Provide preventative care to residents in skilled nursing facilities

  • Conduct on-site visits and remote monitoring to identify potential health issues

  • Develop and implement personalized care plans to minimize hospitalizations

  • Collaborate with our clinical team to ensure seamless care coordination
    *-document patient information and treatment plans efficiently and accurately

Requirements

  • Active Nurse Practitioner (NP) license in the state of Illinois

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Strong patient-centered approach and commitment to preventative care
