Nurse Practitioner

Job Summary:

Carease Health - IL is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner at Carease Health - IL, you will play a critical role in our comprehensive preventative care program, leveraging Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical health and well-being.

Responsibilities:

  • Deliver high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation

  • Build strong relationships with the facility, maintain a patient-centered approach, and collaborate with the care team to ensure comprehensive care

  • Conduct routine assessments, develop and implement individualized care plans, and provide education and support to patients and their families

  • Participate in quality improvement initiatives and collaborative care management efforts to minimize hospitalizations and enhance patient outcomes

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Master's degree in Nursing or a related field
