Job Summary:
Carease Health - MA is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team of healthcare professionals dedicated to reducing hospitalizations and improving patient care. As a key member of our staff, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home. With a focus on building strong relationships, maintaining a patient-centered approach, and being a collaborative team player, you will be an integral part of our mission to improve the lives of nursing home patients.
Responsibilities:
Provide high-quality preventative care to residents in skilled nursing facilities, including on-site visits and remote work from home
Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) technologies to enhance patient care
Maintain accurate and timely documentation, supported by a dedicated scribe team
Build strong relationships with facility staff, patients, and family members
Remain up-to-date with industry developments, best practices, and evidence-based guidelines
Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
Requirements:
Active NP or PA license (in the state of practice)
1+ year of experience in Geriatric care and Documentation review
Excellent communication, collaboration, and teamwork skills
Masters-level education in a relevant field
What We Offer:
Compensation: $23,000 - $25,000 annually
