Job Requirements Our Registered Nurses provide high quality patient care to patients suffering from the most advanced stage of chronic kidney disease, known as End Stage Renal Disease (ESRD). Nurses are on the front line of patient care, improving the quality of life of every patient. This role is all about the relationships and bond you will create with patients and their families. This is a part-time position. Work Experience Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (BSN); three year diploma from accredited diploma program may be substituted for nursing degree. Registered Nurse License in the state of practice. Current BLS/CPR certification required. Meets all applicable state/licensing requirements. Previous dialysis, ICU, CCU, ER, or Med Surg nursing experience is preferred but not required. The Oncology Nurse Navigator (ONN) uses their oncology-specific clinical knowledge to offer individualized guidance to patients, families and caregivers to foster independence during their complex cancer journey. Using the nursing process, an ONN assesses patient needs and provides education and resources to facilitate appropriate and timely quality health and psychosocial care throughout all phases of the cancer continuum. Create patient specific premedication regiment in collaboration with procedure APP. Obtain IV access and preprocedural lab work. Administered preprocedural procedural medications, titrating according to specific patient needs. Also administer chemotherapy and performs procedures safely and efficiently in accordance with Institute policy. May administer blood transfusions. Work as co-sign to Intrathecal Chemotherapy administered by APP. Assists in emergency nursing procedures necessary for prompt control of changes in patient`s physical condition. Maintains awareness of procedural suite activities and works with the Procedure Suite team to maximize workflow. Keeps abreast of changing techniques in procedural nursing and maintains a high level of expertise in critical care. Responsible for intraprocedural monitoring of patients not limited to maintain active airway, assesses effective circulation and respiration via capnography and telemetry monitoring. Safely recover patients post sedation, perform discharge teaching, and evaluate safety of patient discharge. Coordinate correct research specimen collection with research coordinators. Maintain Joint commission readiness. Provide as needed support for biopsy specimen collection when not acting in a procedural sedation role. This position ensures the appropriate utilization of clinical resources with a goal of a safe and timely discharge for the patient. This role navigates health system services to support effective transitions while advising the team on healthcare industry compliance. The RN Case Manager must be adept at driving throughput metrics, clinical effectiveness, and fiscal responsibility. Job Description: Essential Functions: The RN Case Manager collaborates with the health care team to develop the plan of care and patient flow. Tasks: Reviews all cases within 24 – 48 hours or the next business day of admission/bed placement and each day throughout the stay to facilitate care progression to establish an anticipated length of stay and transition planning needs.
