MDS Coordinator- RN OR LPN

We are seeking an experienced full-time MDS Coordinator to join our dedicated team at Cedars of Lebanon, As an MDS Coordinator, you will play a crucial role in coordinating the RAI process, ensuring the timeliness and completeness of the MDS, CAA's, and Interdisciplinary Care Plan.
Responsibilities:
• Monitor and guide the completion of assessments in a timely manner
• Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtaining receipt of validation
• Schedule and facilitate care plan conferences and monthly QM meetings
• Manage Care Planning Process
Mandatory Qualifications:
• Must hold a Nursing Degree from an accredited college or university
• Current, unencumbered, active RN license or LPN and CPR
• At least 3 years' experience in Long Term Care as a MDS Coordinator
• RAC-CT Preferred
Desired Skills and Qualities:
• Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement
• Excellent written and verbal communication skills and critical thinking abilities
• Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
• Ability to organize and prioritize
