A Brief Overview

The Cardiac Care Unit (CCU) provides intermediate level care to patients with congenital and acquired heart disease as well as those with primary rhythm abnormalities. Patients include medical and surgical cardiac patients with an age range typically spanning newborns to young adults. These patients may include those with non-invasive ventilation, such as CPAP/BiPaP and stable trachs. Some patients require continuous inotropic infusions to support their heart function while others require mechanical support such as ventricular assist devices (VADs). As a unit dedicated to inter-professional collaboration and education, we strive to provide care to our patients and families while on the continuum of their inpatient stay within the Cardiac Center. This is a part-time, rotating shift position.

The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with, and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. Provides age/developmentally appropriate care. Care is delivered through RN leadership for patient care within the framework of family-centered care.

The Clinical Nurse adheres to the organizational and nursing department mission, vision, and values. Clinical Nurses practice in accordance with the ANA Code of Ethics; the ANA Standards of Clinical Nursing Practice; practice standards of applicable specialty nursing organizations; and CHOP developed standards of nursing practice. The professional nurse practices within the defined scope of practice designated by the Nurse Practice Acts of Pennsylvania and/or New Jersey, depending on the assigned work site.

What you will do

  • Clinical Practice
  • Follows the nursing process to deliver safe, compassionate, and culturally effective care.
  • Gathers and recognizes/synthesizes assessment data to determine the patient/family plan of care, pertinent data to be shared with care team, and tasks to be delegated.
  • Recognizes and/or seeks resources to recognize clinical changes that are indicative of patient deterioration; utilizes the CAT team when appropriate.
  • Collaborates with the patient, family and multidisciplinary team to coordinate all facets of patient care.
  • Develops, implements and/or revises the patient’s plan of care on admission and during the course of the hospitalization. Plans of care are outcomes focused and include goals as well as required interventions. Plans are developed in partnership with the patient, family, and the interdisciplinary care team.
  • Care is coordinated across the continuum – understands and utilizes case management resources.
  • Consistently follows evidence based practice bundles and nursing standards/procedures to provide nursing care that eliminates preventable events of patient harm and promotes excellent patient outcomes.

Registered Nurse - 3E, Intermediate Care Unit

Job Description Nemours is seeking a Registered Nurse for our general pediatrics stepdown unit to join our team in Wilmington, Delaware . This position is FULL-TIME, 72 hours biweekly, 12-hr rotating shifts, with every third weekend and holiday rotation . 3 East is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries, and general surgery. 3 East also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air, and suction along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the healthcare team to access a patient’s electronic medical record. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, Oxygen delivery methods, PCA & epidural pumps, wound vacutainers, ventilators and specialty beds. These include, but are not limited to cardiopulmonary resuscitation, tracheal intubation, ECMO, pharmacologic support of circulation, mechanical ventilation, point of care monitoring, CRRT, thoracentesis, and hemodynamic, intracranial pressure, intravascular monitoring. Each patient room has electrocardiographic, pulse oximetry, respiratory, hemodynamic and intracranial pressure monitoring capability, as well as central EKG and respiratory monitoring. Portable monitoring during patient transport is provided. Special equipment includes radiant warmer beds, heating/cooling blanket systems, Mapleson manual ventilation system, end tidal CO2 monitors, ventilators, hemodynamic monitoring systems, intracranial pressure monitoring system, ventriculostomy systems, infusion pumps, blood warmer, continuous renal replacement systems, and peripheral nerve stimulators. Telephone Triage Nurse-Care on Call

Job Description Nemours Children’s Health is seeking an experienced pediatric telephone triage nurse to support our primary care practices in Delaware, Pennsylvania and Florida. This is a casual (8 hours/week), fully remote position that is self-scheduled and requires one shift every other weekend and two holidays per year. Weekdays Hours for the role range from: 7:00am-5pm (D) 4:00pm-12am (AH) Overnight 12:00pm-8:30am No permanent night shifts are included for this role, but must be available to cover up to 3 overnight shifts per year Weekends 7:00am-3:30pm 3:30pm-12:00am Overnight 12:00pm-8:30am Primary Function The Triage Nurse role provides provide primary care patients telephone access to a highly skilled and trained registered nurse who accurately answers their questions and addresses their medical concerns during hours when the primary care offices are open. To provide first line response to patient questions and concerns with escalation to the primary care offices when necessary. To ensure continuity of care, communicates the patient’s condition and plan of care to other health team members. Essential Functions Accurately assesses each patient’s condition by actively listening to callers and asking probative questions. Identifies emergent situations and effectively transfers life threatening calls to appropriate agencies per protocol. Provides appropriate and courteous telephone advice including plan of care and patient education to callers according to approved protocols. Appropriately and accurately documents initial and ongoing telephone conversations with documentation being completed immediately after the call ends. Places outbound follow up calls to caregivers as needed. Accurately communicates information to physician on call as needed either over the telephone or by routing the necessary information to the physician through EPIC. Works autonomously while adhering to organizational standards and core values. Attends and participates in staff meetings and required departmental meetings. Meets the needs of the department by flexing hours as needed based on seasonal changes and call volume. This may include morning, evening, weekend, and midnight shifts. Meets unit expectations for call volumes, call duration and customer service expectations. Appropriately utilizes and maintains Nemours supplied equipment; ability to troubleshoot technology issues to ensure uninterrupted service to patients. Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee’s supervisor. Job Requirements Bachelor’s Degree strongly preferred. Associates Degree prepared candidates must be actively enrolled in a BSN program. Upon hire, the Kids Health On Call nurse must have an active unrestricted Delaware Registered Nurse license to start. Active Pennsylvania and Florida Registered Nurse Licenses must be obtained within 90 days of hire date. BLS from American Heart Association required for all nursing roles. Nurses residing in an interstate compact state (DE, NJ or MD) who have a multistate license for their state of residence will not need to obtain separate PA or FL licensure. Nurses residing in PA will continue to require separate DE and FL licensure until PA begins full participation in the interstate compact. The KHOC associate must not work autonomously until all licensure is obtained and verified- they can shadow and train with a KHOC preceptor, but cannot provide medical advice for patients receiving primary care services in states for which the associate has not yet obtained licensure. 3-5 years of job related experience.
